- The FAA has a requirement for an FAA OGE 450 and 278 Electronic Filing Program for the Office of the Chief Counsel, Ethics Staff. A copy of the Statement of Work (SOW) is attached. This market survey is for information purposes only and information received will be used to determine the acquisition strategy for this procurement. The North American Industry Classification System (NAICS) code for this procurement is 541511 Custom Computer Programming Services, Size Standard $25 million. When responding to this market survey, please submit the following to Katherine M. Williams at katherine.m.williams@faa.gov. 1. Performance Capability Statement (no more than 3 pages in length) which addresses past performance information confirming your ability to successfully complete the requirement. Past Performance information must include project title, agency name, order/contract number, project value, duration, point of contact including name, email and phone number and a brief description of the work performed. 2. Business Declaration Form (attached) and a copy of your SBA 8(a) certification letter, if applicable. Responses to this market survey are due June 22, 2012 no later than 4:00pm Eastern Standard Time (EST). Your response should be emailed to Katherine M. Williams at katherine.m.williams@faa.gov. Reference the title and solicitation number in your response. This market survey shall not be construed as a commitment of any kind or a contract. This is not a Screening Information Request (SIR) or a Request for Proposal (RFP). The FAA is not seeking or accepting unsolicited proposals. The FAA will not pay for any information received or costs incurred in preparing the response to the market survey. Any cost associated with responding to this market survey is solely the interested vendor's expense.
