- Description
- (i) This is a combined synopsis/solicitation for commercial items in accordance with the Format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and written solicitation will not be issued. (ii) Request for Quotation (RFQ) FA5270-12-Q-0006 is a combined synopsis/solicitation for commercial items. This combined synopsis/solicitation is issued as an RFQ. Only written quotations will be accepted. Oral offers will not be accepted. To be eligible to receive an award resulting from this solicitation, contractors must be registered in the Department of Defense Central Contractor Registration (CCR) database, NO EXCEPTIONS. To register, you may apply through the Internet at http://www.ccr.gov. (iii) This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58 and through Department of Defense Acquisition Regulation Change Notice 20120420. It is the contractor's responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at: http://farsite.hill.af.mil/vffar1.htm. (iv) This procurement is being issued as firm fixed price, and unrestricted. North American Industrial Classification Standard: 621399, Offices of All Other Miscellaneous Health Practitioners. A single contract award will be made as a result of the evaluation of quotations received for/from this solicitation. (v) The requirement is for four (4) dental assistants at Kadena Air Base, Japan. This commercial service is being acquired in accordance with the attached performance work statement (PWS). ITEM DESCRIPTION: CLIN 0001: Four (4) Dental Assistants for 18th Dental Squadron. The contractor shall perform in accordance with the attached performance work statement. Total hours for each individual: 1000 hrs/for a 6 month period (1 July 2012 through 1 Jan 2013) of performance. QUANTITY: 4000 UNIT OF ISSUE: Hours *Please provide your quote broken-down to price per hour for a total of 4000 hours. (vi) This requirement is currently FUNDED. (vii) The period of performance of this requirement is 1 July 2012 through 1 Jan 2013. Responsibility and Inspection: the contractor is responsible for the performance of all inspection requirements and Quality Control (QC). (viii) The provision at FAR 52.212-1 Instructions to Offerors Commercial Items, applies to this acquisition. All companies must be registered in Wide Area Workflow (WAWF) at https://wawf.eb.mil at time of quote submittal. All companies must be registered in the Central Contractor Registration Database at www.ccr.gov in order to be considered for award. The Government will not provide contract financing for this acquisition. (ix) The provision at FAR 52.212-2(a) Evaluation--Commercial Items, applies to this acquisition. Evaluation for award will be based on lowest price capable of meeting the terms of the PWS. The Government will award a firm-fixed price contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be most advantageous to the Government, price and other factors considered. A contract award shall be based on ‘All or None' and ‘Best Value' to the Government. Best value can include but is not limited to price, delivery schedule, and technical capability. (x) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to the acquisition. Payments shall be made by electronic funds transfer via WAWF. Inspection/Acceptance shall be performed by the Government. The Government may require repair or replacement of nonconforming supplies or re-performance of nonconforming services at no increase in contract price. (xi) The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Item applies to this acquisition. In addition, the following provisions and clauses apply under 52.212-5: FAR 52.222-19 Child Labor-Cooperation with Authorities and Remedies; FAR 52.225-13 Restrictions on Certain Foreign Purchases; FAR 52.232-33 Payment by Electronic Funds Transfer--Central Contractor Registration. Concerns, issues, disagreements, and recommendations that cannot be resolved at the Squadron Ombudsman level may be brought by the concerned party for further consideration to the MAJCOM Ombudsman, Katherine Stockton, HQ PACAF/A7KP, Hickam AFB, HI 96853-5427, phone number (808) 449-8570, fax: (808) 449-8571** (xiii) The clause at DFAR 252.212-7001 Contract Terms and Conditions Required to Implement Statues or Executive Orders applicable to Defense Acquisition of Commercial Items applies. In addition the following clauses apply under 252.212-7001: DFAR 252.232-7003 Electronic Submission of Payment Requests; DFAR 252.247-7023 Transportation of Supplies by Sea. (xiv) There is no assigned DPAS rating. (xv) There are no numbered notes that apply to this requirement. (xvi) Quotations are due no later than 22 June 2012 at 1600 Japan Standard Time. Answer for the following questions: a) Does the Government anticipate the contract will be extended beyond the 6 month period? ----- ANSWER: The contract will not extend past the 6 month period. b) Are incumbent contractors in place or is this a new requirement? ----- ANSWER: No, new contract c) If this is a follow-on contract, what is the current price? ----- ANSWER: No, this is a new requirement. d) Once the 6 months contract period expires, does the Government intend to convert the contractors to GS Employees? ----- ANSWER: No, this is for only the period of service specified in the solicitation. e) There is no mention of a relocation allowance, does the Government intend to issue a separate CLIN for the Relocation or should this be factored into the cost of our bid? ----- ANSWER: This should this be factored into the cost f) Is there names of countries that will be eligible to participate? ----- ANSWER: Terrorist nations as defined by the Federal Acquisition Regulations, Defense Federal Acquisition Regulations, and Air Force Acquisition Regulations are not eligible. *Note* you must follow and meet the full instructions in the solicitation to be considered for award. g) Is there an estimated budget for this purchase? ----- ANSWER: the Government is not allowed to disclose in accordance with federal regulations. h) Is there any extensions of bidding deadlines? ----- ANSWER: no exceptions will be made unless specifically addressed in an amendment to this solicitation i) Are there pre-bid meeting minutes? ----- ANSWER: no, this requirement was not deemed to not require a site-visit.
