Description

The Louis Stokes Cleveland VA Medical Center is seeking potential sources for a Orthopedic Surgeon. The Orthopedic Surgeon shall perform the following clinical services, which include, but are not limited to the following: a.Clinical: 1. Performance of Spine related patient examination in a clinic environment, 8 hours/week. 2. Performance of Spine related surgical procedures in an operative environment, 10 hours/week. 3. Post-operative evaluation, care, procedure evaluation and clinical consultation regarding complications of procedures performed. 8 hours/week. b.Documentation: physician shall document attending supervision of fellows and residents and involvement in patient care via procedure, attending, transfer, and progress notes using the VA's Computerized Patient Record System. All documentation shall be completed in accordance with VAMC Medical Service policy and procedures. c.Health and Safety - Follows all VA Medical Center and Medical Service's guidelines regarding infection control and safety. 2 QUALIFICATIONS: 2.1 Contractor employee(s) (Orthopedic Surgeon) assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. They must: a.Be Board Eligible or Board Certified in Orthopedic or Neurological surgery. b.Successful fellowship in spine surgery; c.Have a current, unrestricted Medical License, and d.Be credentialed by the VA to practice medicine and invasive spine procedures at the Louis Stokes Cleveland VA medical Center. e. Be a United States Citizen f. Be ACLS Certified g. Submit verified credentials sufficient for recommendation of clinical privileges by the Professional Standards Board. 2.2 In addition to the professional credentials listed above, the Contractor employee(s)(Spine Surgeon) shall have performed the following surgical procedures and case minimums: Lumbar Decompression and Fusion: 150 cases Anterior Cervical Discectomy and Fusion: 150 cases Corpectomy and Fusion: 150 cases Cervical Laminectomy and Fusion: 50 cases Cervical Laminaplasty: 50 cases Open Reduction and Internal Fixation of Spinal Fracture Lumbar, Thoracic, or Cervical: 50 cases Open Reduction and Internal Fixation of C2 odontoid Fracuture: 10 cases Vertebroplasty or Kyphoplasty: 10 cases 2.3 The qualifications of all personnel shall be subject to review by the VA Chief of Staff and approval by the VA Facility Director. This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, hubzone, 8(a), small disadvantage business, small business, or large business) relative to NACIS 62111 (size standard of $17.5M. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company name, a service capability statement, three customer references to which you have provided similar services to, copies of applicable licensing, product catalog, address, point of contact, and the size of your business pursuant to the following questions: (1) Is your business large or small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified hubzone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business? Responses to this notice shall be e-mailed. Responses to this sources sought synopsis are not considered adequate responses to any future solicitation announcements.
