This is a Source Sought Notice representing a market research survey and is not a Request for Quotation. NO SOLICITATION DOUMENTS EXIST AT THIS TIME. This Notice is to assist the VA in determining sources. Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub-zone, 8(a), Small Disadvantaged and Woman-Owned Small Business are encouraged to participate. This Sources Sought Market Survey is seeking to identify Offerors able to provide the items below: 1. IN TOUCH, FL27 - INTOUCH ZOOM DRIVE 2. PENDANT MOTION ONLY 3. DOCKER CABLE PAPERWORK REQUIRED 4. XPRT - SEMI-INTEGRATED 5. SEMI-INTEGRATED NYLON COVER (XPRT) NAICS Code 339112 is proposed for this acquisition and small business size standard is 500 employees. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Please place "Attention: Sources Sought - CRC beds" in the subject line of your email. This notice is to assist the VA in determining sources only. Send your response to: frank.gamboa@va.gov by June 28, 2012. Interested Vendor's shall include the following information in their responses: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates your company can provide the supplies/service. 3) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB), (g) State whether your firm is registered with the Central Contractor Registration (CCR) at http://www.ccr.gov and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE any future solicitation could only be awarded to a contractor who is registered in CCR and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry.
