- Service FAR Clause Wage Rates Contractor shall furnish all labor, materials, equipment, transportation; and supervision necessary to perform grounds maintenance for the U.S. Coast Guard as identified in accordance with the Scope of Work (Listed below). I have all attached a copy of the Service FAR Clauses and Wage Rates. Scope of Work: 1.0. INTRODUCTION The Coast Guard is procuring Mosquito Control Services as described in the following paragraphs for the Coast Guard Station Grand Isle, LA. The Performance Work Statement (PWS) is in coherence with the direction in which the Coast Guard is procuring contracts to ensure consistent performance and be result-oriented. 2.0. BACKGROUND 2.1. Mission: The mission of Coast Guard Station Grand Isle is to maintain a safe, clean and healthy environment for all military/civilian personnel and members of the public. To accomplish this mission, it is essential that the personnel, properties and facilities of CG Station Grand Isle have qualified and effective Mosquito Control Services. The purpose of this PWS is to provide support to CG Station Grand Isle to accomplish its mission. 3.0. SCOPE 3.1. The objective of this effort is to acquire all personnel, equipment, tools, transportation, management, supervision, and other items and services, necessary to control and prevent the infestation of mosquitos. The service requirements and performance objectives relate directly to mission essential items as identified in the PWS and the Government Monitoring Functions providing for the highest level of service delivery performance critical to mission success. The Contractor shall use the PWS to accomplish the functions/tasks generated for this effort. The Contractor will be responsible to spray the entire ground of 26 acres. On the 26 acres we have 33450SQ feet of building space that will need to be sprayed inside for pest control. 3.2 The contractor shall be responsible for restoring any damaged property or equipment caused by the contractor or the contractor's employees. The restoration shall be to its original condition at no additional cost to the Government. 4.0. APPLICABLE DOCUMENTS 4.1. The Contractor shall comply with all applicable federal, state, local and municipal laws, regulations and requirements; all industry standards/references; and, all Government and Coast Guard Regulations, attachments and exhibits listed in the table below applicable to the performance of the contract and to contractor personnel. The contractor shall instruct and guide its employees regarding any directives provided by the Coast Guard. 4.2. The Government will provide all publications listed upon request. Supplements or amendments to listed publications may be issued during the life of the contract. The Contractor shall immediately implement those changes, which result in a decrease, or no change in the contract price. Upon completion of the contract, the Contractor shall return to the Government all issued publications and any unused forms. TECHNICAL REQUIREMENTS 5.1. General 5.1.1. Management: The Contractor shall manage the total work effort associated with the services required to assure fully adequate and timely completion of these services. Included in this function will be a full range of management duties including, but not limited to, planning, scheduling, report preparation, establishing and maintaining records and quality control. 5.1.2. Permits and Licenses: Prior to starting work under this contract, the contractor shall obtain the necessary permits and licenses in accordance with the requirements of the municipality, county, and state and shall maintain such licensing throughout the term of this contract. 5.1.3. Storage Area and Controls: The Contractor shall not store chemical materials on Coast Guard property. The Contractor shall comply with all federal and local regulations pertaining to the storage, transport and usage of pest control chemicals. 5.1.4. Pesticides. All pesticides shall be processed, handled, and applied in strict accordance with the manufacturer's label instructions. All pesticides used by the Contractor shall be registered with the U.S. Environmental Protection Agency (EPA), the agency responsible for enforcement of the Federal Insecticide, Fungicide and Rodenticide Act (FIFRA), and the State of Louisiana for the intended use. The Contractor shall maintain a label book of all pesticides used, and have it readily available at all times. A copy of the label book and the material safety data sheet for each pesticide to be used shall be provided to CG Station Grand Isle. 5.1.4.1. Pesticide Application. Certified personnel shall make all pesticide applications. The Contractor shall apply pesticides in accordance with manufacturing label instructions and the pesticide application requirements specified herein. When control and/or equipment are specified the Contractor may use any or all of the listed methods necessary to obtain the acceptable level of control. 5.1.4.2. Pesticide Disposal. The Contractor shall dispose of all excess pesticides, unusable pesticides, pesticide containers, equipment rinse water, and rinse water in accordance with the label instructions and all Federal, State and local regulations. 5.1.4.3. Pesticide Spills and Decontamination. The Contractor shall be responsible for properly cleaning, decontaminating and reporting pesticide spills. 5.1.4.4. Improvements in Technology. As technology improves and new materials and methods become available, it may become necessary to alter the specifications of this contract to accommodate such changes which would reduce reliance on chemicals, or use safer materials. 5.1.5. Safety 5.1.5.1. Measures. The contractor shall take all reasonable precautions in the performance of the work under this contract to protect the environment, health, and safety of employees and of members of the public and shall comply with all applicable federal environment, health, and safety regulations and requirements (including reporting requirements, directives, and orders) of the Coast Guard. It will be the responsibility of the contractor to assure that proper techniques and procedures are followed and precautions taken to prevent injury to any person or property as a result of the work required. The contractor shall have equipment on hand to adequately protect his personnel. 5.1.5.2. Plan. The contractor shall establish, implement and maintain and submit a Safety Plan to ensure the safety of contractor and Coast Guard employees. The plan shall be maintained to current Occupational Safety and Health Administration (OSHA) standards. 5.1.5.3. Electrical Equipment. All electrical equipment used by the contractor shall meet OSHA safety requirements and shall be Underwriter's Laboratory approved. 5.1.5.4. Chemical Supplies. The contractor shall use all chemical supplies in accordance with OSHA safety requirements. Material Safety Data Sheets (MSDS), which show the chemical content of each product, must be submitted for chemical supplies proposed for use by the contractor. All chemical supplies used in the performance of this contract are subject to evaluation/approval by the Environmental Health and Safety personnel. 5.1.5.5. Vehicles. The contractor shall assure that all vehicles used in the performance of this contract conform to Coast Guard Station Grand Isle, LA regulations. 5.1.6. Contractor Certification: The Contractor and/or the Contractor's Pest Control Operators (PCO) shall be registered with the Environmental Protection Agency (EPA) and be certified by the Louisiana Board of Health to perform pest control in accordance with the provisions of the Federal Insecticide, Fungicide, and Rodenticide Act (FIFRA) and the Federal Environmental Pesticide Control Act (FEPCA). The Contractor shall present proof of certification prior to starting contract. 5.1.6.1 Treatment Methodology and Products: The contractor and the PCOs shall conform to all federal, state and local regulations governing the performance of pest control work and the use of pesticides. The chemical formulations used in this project shall be only those recommended by the U.S. Public Health Services, the Bureau of Reclamation (BUREC) Reclamation Safety and Health Standards (RSHS), U.S. Fish and Wildlife Service, the U.S. Department of Agriculture, Louisiana regulations and the registered label use for control on the type of pests involved.
