- Attachment B Attachment A RFP SAQMMA12R0114 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation; proposals are being requested and a written solicitation will not be issued. SAQMMA12R0114 is being issued as a request for proposal (RFP). The solicitation document, SAQMMA12R0114 and incorporated provisions and clauses are those in affect through Federal Acquisition Circular 2005-58. This acquisition is 100% 8 (a) small business set aside utilizing NAICS code 488999 with a size standard of $7,000,000. The provisions at 52.212-1 and 52.212-2 apply to this acquisition. Offerors are advised to include a completed copy of the provision at 52.212-3. The terms and conditions applicable to this action are: 52.212-4, Contract Terms and Conditions-Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items. The Department of State's, Bureau of Logistics Management seeks to award a single award Indefinite Delivery Indefinite Quantity (IDIQ) contract that provides assistance/advising Government employees transferring to or from overseas posts with the shipment of their household effect, privately - owned vehicles (POV), unaccompanied air baggage and consumables, and will advise employees on insurance, customs clearances and related administrative support to the Office of Logistics Operations Transportation Management and Claims. We will establish one-year IDIQ with the option to extend up to four additional years. Additional information is located in the attached solicitation document SAQMMA12R0114, statement of work, Wage Determination, and Past Performance Questionnaire. 1) Proposal must meet all solicitation requirements, such as terms & conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offer being ineligible for award. 2) Past Performance Questionnaire's are to be sent directly to the government contract in this notice from the contractor's reference. Electronic submission from reference is accepted. 3) Proposals are to be provided in two (2) Volumes Volume I - Technical Proposal - submit an Original, plus 4 hard copies Volume II - Price Proposal - submit an Original, plus 2 hard copies Both Volumes are to be sent electronically and hand carried to the government contact listed below.
