Description

I.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All bidders must also be registered on www.ccr.gov and https://orca.bpn.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). II.This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number VA-246-12-Q-2642. This solicitation is a request for a FAST ULTRASOUND training system to be delivered to the VA Medical Center, Salem, VA which meets the requirements within this solicitation. III.The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. IV.This is a total small business set aside solicitation. The North American Industry Classification System Code is 326299 and the size standard is 500 employees. V.This combined solicitation/synopsis is for purchase of the following commercial supplies which meet the following salient characteristics and requirements: 1.Ultrasound system that is High Fidelity with the capability of performing all of the abdominal structures, heart structures, appendages with true anatomy images. The system must have the capability of recording & time stamping the training session for Debriefing, and the system must allow trainees to use medical center ultrasound equipment. Software which simulates the structures will not be accepted; the ultrasound training system must be constructed with the structures in place (ie structures are located within a mannequin) and capable of being detected with hospital owned ultrasound equipment. 2.The following items, brand name or equal to the listed Blue Phantom products, are requested. If offering equal, provide the manufacturer name, brand, and part number. VIII.The Government will award a firm fixed priced contract to the responsible offeror whose offer, conforming to the requirements in this request for proposal, will be most advantageous to the Government, price and other factors considered. A best value award will be made based upon the following factors: Technical and Price. a.Technical: ability to provide products which conform to the salient characteristics and requirements listed in this solicitation b.Price IX.The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (April 2011), with its quote.
