- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, a written solicitation will not be issued. The solicitation number is 2012-48375. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-46. The associated North American Industrial Classification System (NAICS) code for this procurement is 611710 with a small business size standard of $7.0 million. Proposals are due by July 8, 2012 at 12:00 p.m. The place of performance is 236 11th Street SE, Washington, D.C. 20003 The Centers for Disease Control and Prevention (CDC) intends to issue a sole source, firm fixed price contract award to International Conservation and Education Fund, 236 11th Street, SE, Washington, D.C. 20003, for providing technical expertise, labor, materials and equipment to (1) Identify and train 3 monkeypox education teams, capable of leading discussions, answering questions and conducting pre- and post-viewing surveys of villagers who attend the video screenings; (2) Consult with CDC subject matter experts regarding the preparation of the community outreach and survey tools; (3) Travel to Tshuapa and arrange for community outreach including educational videos in approximately 30 villages; and (4) Provide a detailed personnel plan and schedule for the outreach program to CDC by August 30, 2012. This procurement will be processed under the authority of FAR 13.106-1(b)(1). Only one responsible source and no other sources will satisfy the Agency's requirements because they are experienced in the production and dissemination of educational videos for prevention of zoonotic diseases found in Congo Basin forests; maintain film and video production equipment and trained Congolese staff locally in Kinshasa; and demonstrate the capacity to translate complex zoonotic disease prevention messages into culturally appropriate messages for low literacy settings in Congo, and produce video and training materials in multiple languages. This notice of intent is not a request for competitive quotes/proposals, however, all responsible sources may submit a capability statement with a cost proposal and a completed copy of the provision at 52.213-3, Offerors Representations and Certifications - Commercial Items which, if received by the response date, shall be considered by the Agency. A determination by the Government not to open the requirement to competition based upon the responses to this notice is solely within discretion of the Government. The following provisions apply to this commercial solicitation: 1. FAR clause 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition and a statement regarding any addenda to the provision. 2. FAR clause 52.212-2, Evaluation - Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. If this provision is not used, describe the evaluation procedures to be used. 3. Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. 4. FAR clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. 5. FAR clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. 6. Additional contract requirement(s) or terms and conditions may be required for this acquisition of determined necessary by the Contracting Officer and consistent with customary commercial practices.
