Description
The Portland VA Medical Center, Portland, OR, is considering the development of one or more Basic Ordering Agreements for the provision of sub-acute residential alcohol and drug detoxification services to Veterans served by Portland Veteran's Affairs Medical Center (PVAMC). The level of care will be Medically-Monitored Detoxification in a sub-acute residential setting. C.2 The Contractor shall provide the following Medical Services: C.2.1 A physical exam conducted by an RN at admission. C.2.2 Medically-monitored detoxification from alcohol and drugs C.2.2.1 Detoxification from opioids utilizing buprenorphine and other agents as needed for symptom relief C.2.2.2 Detoxification from alcohol utilizing chlordiazepoxide or oxazepam and other agents as needed for symptom relief C.2.2.3 Monitoring and symptomatic relief from stimulant withdrawal C.2.2.4 Monitoring of chronic medical conditions during withdrawal C.2.2.5 Monitoring for mental health symptoms that emerge during withdrawal C.2.2.6 24-hour Nursing coverage C.2.2. 7 Availability of an on-call Physician 24 hours a day C.2.2.8 Availability of the contract facility Medical Director by phone 24 hours a day for emergencies C.2.3 Urgent Care Clinic services C.2.3.1 Medical management of chronic medical conditions C.2.3.2 Medical management or referral for acute medical conditions C.2.3.4 Medical management of complications of withdrawal C.2.3.5 Urgent Care Clinic will be staffed by a Physician or Nurse Practitioner 3 times per week C.2.4 Addiction Counselor or Rehabilitation Tech support C.2.4.1 Availability of support for monitoring and documenting vital signs and behavior C.3 The Contractor will provide the following Clinical Services: C.3.1 Alcohol and drug counseling, and services below, by certified alcohol and drug counselors: C.3.1.1 Written Biopsychosocial Assessment C.3.1.2 Written Treatment Planning and Discharge Planning C.3.1.3 Individual and group supportive counseling C.3.1.4 Patient education on effects of alcohol and drugs, relapse prevention, and community treatment and housing resources. C.3.2 Availability of acupuncture for those requesting it C.4 The Contractor will provide the following Support Services: C.4.1 Separate sleeping and dining facilities for women C.4.2 Three meals a day, plus snacks and drinks C.4.3 Availability of laundry facilities C.4.4 Personal hygiene items as needed C.4.5 A second set of clothing, ie, scrubs, robes, slippers C.4.6 A bed, bedding, and towels C.4.7 A secure locker and/or wardrobe space for personal property C.S The Contractor will provide access and referral services: C.S.l Priority access to residential detox services will be provided each morning for at least one hour, Monday through Friday. C.5.2 The Contractor will accept same-day referrals as physician staff are available and pre-arranged admission appointments, provided the Veteran has been cleared for sub-acute Medically-Monitored Detoxification, up to the funding limits of the contract. C.5.3 In the event that the funded contract treatment episodes are exhausted, the Contractor will notify the Contract Officer and Clinical Coordinator that the standard non-contract admission procedures will be re-instituted. C.5.4 Contractor will accept medically- and psychiatrically-cleared referrals from the PV AMC Emergency Department, Inpatient Psychiatry program, Substance Abuse Treatment Program, Opiate Treatment Program, and Primary Care Provider. 5.C.5 Contractor will refer Veterans whose medical condition cannot safely be treated in the sub-acute residential level of care to the PV AMC Emergency Department or, in the event of diversion, to the nearest hospital. In a medical or psychiatric emergency or urgent care situation, Contractor will notifY the PV AMC ED prior to returning a patient to the PV AMC. C.5.6 Contractor will refer Veterans who leave early or AMA, as well as those who complete detoxification, to the most clinically appropriate level ofV A substance abuse treatment, to include residential or outpatient substance abuse treatment at PV AMC or another VA facility. C.S. 7 Contractor will notifY the PV AMC Substance Abuse Treatment Program (SA TP) and forward documents to the designated SA TP Detox Liaison person prior to discharge from Contractor's facility. Documents forwarded at the time of referral will include: Assessment, Active Medications sheet, all Progress Notes pettinent to the transfer, and the Discharge Note or Summary. C.6 The Contractor will adhere to all applicable accreditation and certification standards: C.6.1 The Contractor will meet applicable Joint Commission for the Accreditation of Healthcare Organization Life Safety Guidelines. C.6.2 The Contractor will notify PV AMC immediately and in writing of deficiencies in meeting accreditation, certification, or other Federal, State, or Local standards. C.6.3 Contractor will abide by new accreditation or certification standards as they arise during the period of this contract. C. 7 The Contractor will provide for the health and safety of patients and staff: C. 7.1 The building shall conform to the standards of the Life Safety Code (National Fire Protection Association Standard #I 01) in effect on the date of contract award. C.7.2 The Contractor will meet all Federal, State and Local Laws, regulations, and codes pertaining to health and safety. C.7.3 The Contractor will assist PVAMC personnel to conduct periodic inspections of patient and employee safety, security, and sanitation practices throughout the facility at the discretion of the VA. C.7.4 The Contractor will notifY PVAMC immediately and in writing of deficiencies in meeting Federal, State, or Local standards for health and safety of patients and staff. C. 7.5 Contractor will assure the facility is secure during and after business hours by employing staff observation, locked doors, or cameras. C.8 The Contractor will provide for safe storage, administration, and dispensing of medications: C.8.1 The Contractor will provide locked storage for the Veteran's medications C.8.2 The Contractor will maintain a system for monitoring and documenting the Veteran's medication compliance C.8.3 The Contractor will notify the PVAMC Detox Liaison of prescription discrepancies, unanticipated side effects, adverse medical or psychiatric events, or other medication-related problems and will seek reconciliation of such problems. C.8.4 The Contractor will maintain safe procedures for buying, storing, dispensing, administering, reconciling, and disposing of medications. The Contractor will allow inspection by PVAMC personnel ofthese policies, procedures, and facilities. C.9 The Contractor will provide reports and information for Quality Assurance Monitoring: C.9.1 Contractor will submit a list of all admitted Veterans, V A-referred or self-referred, with each patient's name, Social Security number, address, drug(s) of choice, referral information (to and from the Contractor), and program completion status. This will be submitted to the MHND Clinical Consultant quarterly. C.9.2 Contractor will conduct a random sample of 3.0 medical records on V A-referred patients and determine the percent who have been referred to ongoing treatment at a VA Substance Abuse Program. This will be submitted to the MHND Clinical Consultant VA quarterly. C.9.3 Contractor will repmi the percent ofVA-referred patients who complete the detox program. This will be submitted quarterly. C.9.5 Contractor will repmi adverse medical and psychiatric events, such as medication errors, adverse reactions to medication, sentinel events (loss of life or threat of injury to life or limb) involving any Veteran, V A-refen ·ed or self-referred, to the Detox Liaisons within 24 hours. A summary repmi of these events will be submitted quarterly.
