GENERAL INFORMATION: Sources Sought Notice (Market Survey), NAICS Code: 621399 Release Date: 11 July 2012 Response Date: 19 July 2012 NLT 1400 hours EST via email to terry.hembree@us.army.mil Contracting Office: US Army Medical Command North Atlantic Regional Contracting Office, Fort Knox, KY Description of Medical Services: To provide ancillary health care support for Camp Atterbury, IN during the base period from 1 October 2012 thru 30 September 2013; Option Period 1 from 1 October 2013 thru 30 September 2014; Option Period 2 from 1 October 2014 thru 30 September 2015; Option Period 3 from 1 October 2015 thru 30 September 2016; Option Period 4 from 1 October 2016 thru 30 September 2017. Services are as follows: Physician Assistant, 10,440 hours per year; Nurse Practitioner, 4,176 hours per year; Licensed Practical Nurse, 10,440 hours per year; Pharmacy Technician, 2,088 hours per year; Radiologic Technician, 4,176 hours per year; Medical Laboratory Technician, 4,176 hours per year; Optometrist, 2,088 hours per year; Licensed Clinical Social Worker, 4,176 hours per year; Contract Manpower Report (CMR), 1 report per year. Only the actual hours worked can be billed to the Government. This announcement constitutes a Sources Sought Synopsis (market survey). This is not a request for quote (RFQ) or a request for proposal (RFP). This announcement is for information and planning purposes only and is not to be construed as a commitment by the Government, implied or otherwise, to issue a solicitation or award a contract. The Government will not pay for any cost incurred in responding to this announcement. Any information submitted by respondents to this Sources Sought Synopsis shall be voluntary. The purpose of this announcement is to determine interest and capability of potential of the following small business concerns: 8(a); HUBZone; Service-Disabled Veteran; and Woman or Women Owned small businesses. The Government may use information received from these sources to make an appropriate acquisition decision. Interested parties must respond to this notice NLT 19 July 2012, NLT 1400 hours.
