Description
The WALTER REED NATIONAL MILITARY MEDICAL CENTER (WNNMC) Acquisition Management Department, Bethesda, MD on behalf of The INFO TECH DEPT intends to awared a procurement for 001- POP 8/15/2012-08/14/2013 FORESCOUT, DATA SYSTEM HARDWARE AND IMMIX TECHNOLOGY MAINTENANCE (NEW) ACTIVECARE ADVANCED SUPPORT (24X7) FOR COUNTERACT<=4000 DEVICES COPPER 002- ACTIVECARE ADVANCED SUPPORT (24X7) FOR COUNTERACT ENTERPRISE MANAGER FOR 10 OR LESS COUNTERACT APPLIANCES The proposed contract action is for supplies or services for which the government intends to solicit and negotiate. This notice is intent for a competitive bid. The government will compete this procurement based on responses to this notice and the discretion of the government. PLEASE READ THE STATEMENT OF WORK LISTED BELOW. ActiveCare Advanced Support - Service Level Terms Terms Annual Annual Software Upgrade Yes Yes Support Website Yes Yes ForeScout Technical Support Center 8 X 5 (Pacific) 24 X 7 X 365 Support Response 3 Business Hours 3 Hours Advanced Hardware Replacement 5 Business days Not guaranteed for non US address 2 Business Day 24 Hours if notified before 4:00PM Eastern guaranteed for a non US address ActiveCare Service Summary Service Provided To Purchaser of ForeScout Products Term One Year, renewable per annum,; Hardware Warranty is valid for 36 months after which only software is covered Prerequisites End Customer must update appliance with current software version. ForeScout will only supply support for two (2) prior software releases. This applies to point revisions as well as major releases. ForeScout Receivables Purchase Order • End Customers must provide serial number of each appliance covered under the support contract. • If the Advance Hardware Replacement is delivered, end customer must return ship the replaced defective appliance(s) within seven (7) calendar days of receipt of Advance Hardware Replacement. • If the defective appliance is not returned, a replacement fee equal to the full non-discounted price will apply for any appliances not received by ForeScout within 14 days of Return Materials Authorization (RMA) initiation. • Completed Customer Information Sheet, with current address of the physical location of the appliance covered under ActiveCare program. ForeScout Deliverables • Password for secure access ForeScout Support Website. • Invoice upon contract initiation/renewal. Additional Costs and Fees • Replacement fee equal to full non-discounted price of the appliance will apply for any units not received by ForeScout within 14 days of RMA initiation. • Customer must pay any customs and duties associated with product RMA procedures. • Costs associated with any items not returned when returning an appliance, e.g. cables, bezel, rack mounting kit, or any other item listed on the enclosed packing slip. End Customer Responsibilities • Obtain RMA numbers from ForeScout, package and ship according to instructions; return appliances subject to Advance Hardware Replacement in the required timely manner or pay related fees. • Pay for any customs and duties associated with appliance RMA procedures. • Maintain appliance. ForeScout will provide support for the current version plus two (2) prior releases within 12 months of the version release date. Current version plus one (1) release will always be supported. Regardless of release date; problem resolution may require upgrade to current version. • Verify operation of all telephone circuits and digital services for support response escalation services. • Provide accurate physical site location of the appliances, contact and specific appliance information. Notify ForeScout of any equipment moves. • Endeavor to place all ForeScout appliances under the same service agreement, i.e., all appliances at same location should be at similar service levels. • Maintain personnel with adequate technical expertise and training to assist ForeScout in providing troubleshooting and problem resolution. Reseller Responsibilities • If ActiveCare is sold to an end customer through the reseller channel, the reseller must register and maintain ALL appropriate appliance/software version and end customer information with ForeScout. Exclusions This service does not cover the following: • Non-ForeScout updates or modifications to appliance. • Altered appliances, except as authorized by ForeScout. • Appliances installed, operated, or maintained not in accordance with specifications supplied by ForeScout. • Appliances subjected to unusual physical or electrical stress, misuse, negligence or accident, or used in ultra-hazardous activities. • Non-mandatory hardware changes. • Out of revision appliances, i.e., appliances shipped more than 36 months prior to contract initiation. Rack mounted hardware and components will be supported for a maximum of 36 months from date of shipment, desktop hardware and components will be supported for a maximum from date of shipment.
