Past Performance Contract Profile Outline of Transition Plan J 9 Acronym List J 8 Resume Format Template J 7 Invoice Summary Report J 6 Public Voucher Standard Form 1034 J 4 Past Performance Questionnaire J 1 SF3881 Solicitation SAQMMA12R0261 for Accident and Sickness Program for Exchanges for ECA Bureau. This is a combined Synopsis/Solicitation in accordance with the format in FAR Subpart 12.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; SAQMMA12R0261 is being issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition circular. This solicitaiton is a Service-Disabled Veteran-Owned Set Aside only. The Department of State's, Bureau of Educational and Cultural Affairs (ECA), is soliciting proposals for the Accident and Sickness Program for Exchanges (ASPE), in accordance with the attached Solicitation and Statement of Work. It is anticipated Department of State will award one contract with the option to extend up to four additional years. Additional information on the type of work ECA requires from vendors, specific information and how to respond can be found in the attached Statement of Work and Solicitation documents. All questions should be sent to borekMP@state.gov by July 23, 2012 at 12:00(NOON) (EST). All proposals are due by August 9, 2012 by 12:30PM (EST). No phone calls will be accepted. PLEASE NOTE: Please hand carry to deliver hard copies of proposals to: Maureen Borek, U.S. Department of State, Office of Acquisition Management, 1701 N. Ft. Myer Drive, Arlington, VA 22209.
