Photos Drawings Full Scope of Work with Bid Sheet Solicitation/Request for Proposals Monthly Man Hour Exposure Report - Excel Version Monthly Man Hour Exposure Report Accident Investigation Report Activity Hazard Analysis Form Abbreviated Accident Prevention Plan Checklist - Excel Version Abbreviated Accident Prevention Plan Checklist Safety Guide SCOPE OF WORK ITEM 1 SAFETY PLANS AND SUBMITTALS SAFETY REQUIREMENTS: The contractor shall follow all Safety Requirements detailed in the Unified Facilities Guide Specifications (USACE UFGS 01-35-26, February 2012), attached, and applicable Safety & Health Requirements Manual per EM 385-1-1. EM385-1-1 is available on-line at: http://140.194.76.129/publications/eng-manuals/EM_385-1-1_languages/EM_385-1-1_English_2008/toc.html SAFETY SUBMITTALS: Abbreviated Accident Prevention Plan (APP) Completed Abbreviated APP Checklist (blank attached) Activity Hazard Analyses (AHA, blank form attached) Proof of qualifications for Competent Persons/Site Safety and Health Officer(s) (SSHO) to include o A resume detailing work on similar projects. o Copy of the certificate for 10-hour OSHA construction safety course. o Competent person training as needed.  All safety submittals shall be provided to the government within 14 calendar days after contract award. SUBMIT REPORTS AS REQUIRED: Accident Reports via Eng Form 3394 Accident Investigation Report (blank attached) Monthly Man-Hour Exposure Reports by the 5th day of each month (blank attached) Work shall not begin until the APP has been submitted to, and accepted by, the USACE Buffalo District Contracting Officer or COR. In the APP, the Contractor shall avoid providing material of a general nature not specifically related to this project or site. Previous versions of existing off-the-shelf APP are not acceptable. The APP shall be prepared following the format found in USACE publication U.S. Army Corps of Engineers (USACE) Safety and Health Requirements Manual EM 385-1-1, Appendix A. The following documents shall be considered in the preparation of the APP: USACE EM 385-1-1 (15 Sep 2008) Safety and Health Requirements Manual; UFGS-01 35 26 (February 2012) Governmental Safety Requirements; USDOL OSHA 29 CFR 1926 Safety and Health Regulations for Construction; USDOL OSHA 29 CFR 1910 General Industry Safety and Health Standards. Portions of the submittals that are disapproved shall be promptly corrected and re-submitted. If additional work items are added to the contract work the APP and AHA shall be amended as necessary and approved prior to the start of the additional work items. A SSHO shall be present on-site whenever project-related construction work is being executed. The contractor shall permit safety inspections of all work being performed. Payment for this item will be in one lump sum. ITEM 2 WATERTIGHT DOORS FOR PILOT HOUSE: Replace the two doors of the pilot house with new watertight doors. Make such modifications to the pilothouse as are required for compatibility with the new doors. The new doors shall be provided with locks, keyed alike, and two sets of keys shall be provided for the locks. All work shall be suitable for vessels in Great Lakes service, and in compliance with ABS Rules for Building and Classing Steel Vessels. All new construction and any modified areas of the pilothouse or other areas of the vessel where paint is disturbed as consequence of the work shall be prepared and painted to match the existing vessel in accordance with the PAINT SCHEDULE, GENERAL NOTES, and COLOR CHART shown on Drawing 533-D406-01 of the Record Drawings provided as an attachment to this Contract. With the bid, provide such schematic drawings, sketches, and product literature as required to adequately describe the nature of the work for bid evaluation purposes (technical acceptability). After award, and prior to starting work, provide detailed drawings of the proposed work, certified and approved by the American Bureau of Shipping (ABS). After installation, perform such testing of the watertightness of the doors as is required by ABS standards, and provide certification from ABS of successful testing. Payment for this item will be in one lump sum. ITEM 3 WATERTIGHT ENGINE ROOM DOORS AND HATCH: Replace the engine room doors and hatch with new watertight doors and hatch or equivalent entry system. Make such modifications to the engine room enclosure as are required for compatibility with the new doors and hatch. The new doors and hatch shall be securely lockable, and two sets of keys shall be provided for any new locks. All work shall be suitable for vessels in Great Lakes service, and in compliance with ABS Rules for Building and Classing Steel Vessels. All new construction and any modified areas of the engine room enclosure or other areas of the vessel where paint is disturbed as consequence of the work shall be prepared and painted to match the existing vessel in accordance with the PAINT SCHEDULE, GENERAL NOTES, and COLOR CHART shown on Drawing 533-D406-01 of the Record Drawings provided as an attachment to this Contract. Payment for this item will be in one lump sum. ITEM 4 SCHEDULE: All work shall be complete no later than 6 weeks after the date of Contract Award. PLEASE SEE THE ATTACHED SCOPE OF WORK FOR MORE DETAILED INFORMATION, INCLUDING BID SHEET, BID SUBMITTAL REQUIREMENTS, SUBMITTALS, AND TECHNICAL PROVISIONS. BID EVALUATION: This Contract will be awarded to the lowest priced technically acceptable proposal. Technical acceptability will be based on the Government's evaluation of the schematic drawings, sketches, and product literature provided with the bid." The Government will determine technical acceptability based on whether the work shown will be a. Watertight b. Compatible with normal vessel operations (requiring minimal or no modifications to the existing vessel, preventing implementation of designs that create time consuming and/or difficult to use doors) b. Durable (50 year expected life with the exception of replaceable gaskets or seals) All three criteria must be met! Note: Wage Determination for vessel included in solicitation, but upon award, contract will include wage determination with job categories for the location of the winning contractor, as this work will be done at the contractor's facility.
