- STATEMENT OF WORK, DATED 19 JULY 2012 EVALUATION CRITERIA_PROVISIONS_CLAUSES PAST PERFORMANCE QUESTIONNAIRE •a. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. •b. Solicitation A12084 is hereby issued as a Request for Proposal (RFP) and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-58. •c. Set-Aside and NAICS Code: This acquisition is Unrestricted and open to all companies operating under NAICS Code 541720. •d. Description of Requirement and items: The Department of the Treas ury intends to issue a Firm Fixed Price contract for the implementation of a research project on technology-based financial capability tools. This research project will contribute to the Department of the Treasury's mission of promoting the financial capability of Americans. The Office of Financial Education, Financial Access and Consumer Protection (OFEFACP) is responsible for policy development on wide array of issues concerning financial consumers, including policies that enhance personal financial decision-making. While the emerging field of financial capability tools is still young, Treasury recognizes the potential that these services can have for enabling consumers to make better informed financial choices. This procurement is designed to develop knowledge about this nascent field and to support OFEFACP's ongoing policy work on related topics. It includes two primary tasks. The first is a field survey to identify the current financial capability tool landscape. The second is to up to ten quick turn-around reports for the office on this topic as specified in the attached Statement of Work (SOW) entitled, "Support of Treasury Initiative on Policy Development, Research and Evaluation of Technology-Based Financial Capability Tools," dated 19 July 2012 (Attachment 1). In addition to the required information required under FAR 52.212-1, offerors shall also provide the following information in their proposal: •· Company Name and Address •· Company POC & Title, Telephone No., and Email Address •· Completed Representations and Certifications or ORCA printed document in accordance with FAR 52.212-3 •· Discount Payment Terms if other than Net 30 •· Pricing for each Contract Line Item The following Contract Line Items are associated with this requirement: CLIN DESCRIPTION OF SERVICES 0001 BASE PERIOD - (1 Year from Date of Award) The contractor shall provide the necessary skills and labor to perform the tasks associated with this requirement as specified in the SOW entitled, "Support of Treasury Initiative on Policy Development, Research and Evaluation of Technology-Based Financial Capability Tools," dated 19 July 2012. 0002 OPTIONAL TASK- (3 Months from End of Base Period) The contractor shall provide the necessary skills and labor to perform the tasks associated with this requirement as specified in the SOW entitled, "Support of Treasury Initiative on Policy Development, Research and Evaluation of Technology-Based Financial Capability Tools," dated 19 July 2012.
