Description
Synopsis: NOTICE OF SOURCES SOUGHT / REQUEST FOR INFORMATION (RFI). This is a Sources Sought Synopsis in support of a market survey conducted by Department of Veterans Affairs (VA) to determine the interest and capability of potential sources. This synopsis is for informational and planning purposes only and does not constitute a solicitation, a synopsis of a solicitation, or a commitment on the part of the Government to later publish a solicitation. The Government will not reimburse the costs of preparing or submitting a response to this synopsis. Any information submitted in response to this sources sought / RFI is strictly voluntary. Companies may provide a profile normally provided to the general public. Any questions should be submitted to the contract administrator listed below. VA is attempting to determine potential sources capable of fulfilling a need for two (2) Cardiopulmonary Perfusionist. These services shall be available seven (7) days a week, twenty-four (24) hours a day, 365 days per year by the Contractor to Department of Veterans Affairs Medical Center, 3710 SW US Veterans Hospital Road, Portland, Oregon (hereinafter called PVAMC). This service requires an on-call individual/s with an out-of-hospital response time 30 minutes from the time of receipt of notification that services are required. The potential Candidate must be Board certified by the American Board of Cardiovascular Perfusion. Only contractors with proven experience providing this type of service at the PVMAC or another VA medical center will be considered. The Government request all interested offerors submit one copy of their capability responses and cost estimates, by email no later than 4:00 p.m. PT on July 25, 2012 to the email address below. The response should be a profile normally provided to the general public. All respondents must supply the following: Company name, address, point of contact for information, size of business, US or foreign owned, and Federal Supply Schedule number, if applicable, and Service Disabled Veteran Owned Small Business status (If Applicable). This is a request for information only. Funds are not currently available. Qualified companies, who provide responses to this Sources Sought, will be afforded an opportunity to provide a quotation in future requirements.
