Description
Attachments 1-4 THIS IS A TOTAL SMALL BUSINESS SET-ASIDE. This is a combined synopsis/solicitation for Contractor to install replacement street lights at the USPHS Indian Health Service - Chinle Comprehensive Health Care Facility, Chinle, AZ, 86503. The Government reserves the right to issue a single award or multiple awards to the contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following factors will be considered: past performance and price reasonableness. The socio-economic status of an offeror may also be considered, should one or more quotes represent the best value. Any offer over $36,000.00 will not be considered. The Service Contract Act applies to this combined synopsis/solicitation. Contractors performing on service contracts in excess of $2,500 to which no predecessor contractor's collective bargaining agreement applies shall pay their employees at least the wages and fringe benefits found by the Department of Labor to prevail in the locality or, in the absence of a wage determination, the minimum wage set forth in the Fair Labor Standards Act. For this combined synopsis/solicitation the Contractor shall meet the minimum required wage determination as stated in the U.S. Department of Labor - Wage Determination No.: 2005-2023, Revision No.: 16, Date of Revision: 06/13/2012, see attachment. Contractor shall furnish all necessary labor, materials, supplies, tools, equipment, and transportation to remove existing street lights, prep area for install, install QTY (8) two head and QTY (7) one head street lights. Contractor shall also certify the installation to all local, state, and federal codes. Services shall be in accordance with the attached Scope of Work. The new Street Lights shall be in strict accordance with the Specifications provided in order to fit existing concrete footing and embedded hardware; Site Map and Photographs of existing are attached.
