- Combined Synopsis/Solicitation Assessment of Parking Garage Damages This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. Solicitation Number VA248-12-I-2078-2 is being issued an invitation to bid (IFB). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 05-59. NOTICE: This is a service-disabled, veteran-owned small business set-aside. Prospective contractors are cautioned that any bid or proposal submitted in response to this solicitation must meet the criteria identified by 38 CFR, Part 74. Any person, persons, or business entity suspected of misrepresenting itself for the purpose of securing a government contract may be criminally investigated and prosecuted for fraud against the United States of America. Parties found misrepresenting their status also risk debarment from further government contracts. NAICS - 541330 is applicable. Contractor shall provide all necessary labor, materials, tools, equipment, supervision, and specialty expertise for a structural inspection / evaluation of the conditions of the new parking garage after being exposed to an overweight vehicle. Observations indicate, as a result of an overweight vehicle, the seat angles have been vertically displaced. These will be re-set by others. Also included is an appropriate inspection of the structure for additional damage, cracking, and other faults that might have been caused by the overweight vehicle. With a written report of the findings, instructions for necessary structural repairs, if any are required. If none are required it shall be so stated in the report. Issued under the stamp and seal of a Florida registered civil / structural forensic engineer. A copy of drawing Include a cost estimate for the repairs, as needed. All work to be completed within 21 calendar days of the contract date. Contractor shall visit the site to establish all existing conditions and include all implications in their proposal. Al work shall be coordinated, and scheduled with VA's COTR. All work shall take place during VA's traditional administrative hours - 700a to 500p. Saturdays, and Sundays are at the contractor's discretion - as scheduled with COTR - and shall not be cause for changes to the contract. VA holidays shall be NO WORK days for the contractor. VA will NOT receive, unload, transport, or load any contractor materials. Secure storage at VA job sites is NOT available. Contractor shall provide for his own secure storage. All work shall be in accordance with NEC, OSHA, VA standards, and other relevant codes, standard, and practices. VA will not provide any tools, equipment, or materials. VA will assist the contractor with traffic control. The following FAR Clauses apply to this acquisition: FAR 52.212-1 Instruction to Offferors - Commercial Items FAR 52-212-3 - Offeror Representations and Certifications - Commercial Items FAR 52-212-4 - Contract Terms and Conditions - Commercial Items FAR 52-212-5 - Contract Terms and Conditions - Required to Implement Statutes or Executive Orders - Commercial Items Offers are due on or before noon, August 10, 2012 and shall include the following information: 1.The original offer submitted on company letterhead paper and signed by an official of the company. 2.Center for Veterans Enterprise (CVE) certification or re-certification letter dated after December 31, 2011. 3.A completed copy of the representations and certifications at FAR 52.212-3 4.Copy of valid and current Florida Engineering License One hard copy and 1 CD version of the offer should be sent to the Contracting Officer on or before noon, August 10, 2012 to: VAMC - West Palm Beach Attn: Debra Becht - Bldg 12 7305 N Military Trail West Palm Beach FL 33410-6400 All questions concerning this IFB should be e-mailed to the contracting officer at debra.becht2@va.gov.
