- Attachment 2 Pricing Schedule Attachment 1 PWS DHA Med Tech Solicitation SEE ATTACHMENTS FOR SOLICITATION AND ASSOCIATED DOCUMENTS INFORMATIONAL SOLICITATION GUIDANCE: It is important that your company read,understand and submits a proposal that is current, accurate and complete. The list below includes but is not limited to vital information that must be acknowledged as well submitted with your proposal. - This is a Firm Fixed Price Contract and No Conditional Proposals will be accepted, FAR 16.202-1 FAR 15.101-2, solicitation addendum 52.212-1 pages 8-10 and addendum 52.212-2 pages 11-13. - Ensure you submit prices for the Basic year and Option years, Attachment 2 Pricing Schedule. - Ensure you submit all of the required information for all the factors and sub-factors, addendum 52.212-1 and addendum 52.212-2. -Education Requirements-Experience-Certifications & Licensure. FAR 52.212-3, Offeror Representations and Certifications Commercial Items, applies to this acquisition. The vendor must have completed the Online Representations and Certifications Application (ORCA) which is the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (Jun 2008).
