- SOLICITATION NOTICE RFQ VA69D-12-Q-1114_2 SONOGRAPHER FOR ECHOCARDIOLOGY SERVICES The GLAC (Great Lakes Acquisition Center) is soliciting quotations for up to TWO (2) Sonographers for beneficiaries for and at the Jesse Brown VA Medical Center, 820 South Damen Avenue, Chicago, Illinois 60612 for the period of August 13, 2012 through November 16, 2012 in accordance with all specifications, performance work statement, terms and conditions contained therein. All FSS Small Business qualified offerors/firms who believe they can satisfy the solicitation requirements are invited and encouraged to provide their quotes. Offers shall be evaluated for fair and reasonable pricing, technical competency/capability to perform services, experience, availability and satisfactory past performance record. The Government anticipates award of a commercial item task order to the responsible offeror whose offer provides the best value to the Government. VA reserves the right to select the contract employees from different contractors resulting in multiple awards. This is a Firm Fixed Price task order. The size standard for North American Industry Classification System (NAICS) code 621999 ($10.0M). Offerors are reminded to complete all sections of the solicitation. Electronic solicitation may be obtained FREE at the following URL: http://www.fbo.gov/spg/VA/index.html. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments. Offerors should contact VA Central Office (866) 594-0508 regarding any problems downloading the solicitation from the website.
