- THIS IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. This is a notice of intent to award a sole source contract under statutory authority Section 4202 of the Clinger Cohen Act of 1996 as implemented by FAR 13.501(a), "Only one responsible source and no other supplies or services will satisfy agency requirements". 6th Medical Group, Medical Logistics, 3250 Zemke Ave, MacDill AFB, FL 33621, requires Preventive Maintenance for the VISX WaveScan / WaveFront Laser System, SN 77230. Requested service shall be provided to the VISX WaveScan / WaveFront Laser System with following specs: • PC, Monitor and Printer o Windows NT® operating system o Pentium® III processor o Flat-panel, LCD monitor o High-resolution color printer • Isolation Transformer o Medical grade; IEC 601-1 compliant o Electrical ratings: 115/230 V, 50/60 Hz, 5.28/2.64 A o Fuse ratings: 115 V = 6.3 A, 230 V = 3.15 A • Power Supply o DC power source for video cameras (CCDs) and super-luminescent diode (SLD): 120 V~, 50/60 Hz, 20W • Optical Head o Two optical units for the pre-compensation of sphere and astigmatism, adjusted by three stepper motors o Two CCD cameras and a light source (the SLD) o SLD incident power continuously monitored and switched off if defined threshold is exceeded o Class I Laser Product: 785 nm, 65 µW max o Physical dimensions (L, W, H): 18", 13", 24" (46, 33, 61 cm), including base o Weight: 42 lbs (19 kg) • Motorized Table o Rocker switch controls vertical position (range from 630 mm to 1030 mm). o Tabletop holds PC monitor, keyboard and optical head. o Shelves hold PC, printer, isolation transformer, and power supply. o Electrical ratings: 120 V, 50/60 Hz, 6 A o Physical dimensions (L, W): 49", 29" (124, 74 cm); (H):adjustable 25" to 41" (64 to 104 cm) o Weight: 149 lbs (68 kg) o The 6th Contracting Squadron intends to make an award to AMO Sales and Service, Inc., 1700 E. No phone calls please. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority cited above. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive proposals. However, any proposals received by Friday, 17 Aug 2012, 10:00 a.m. EST. will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. NOTE: FAR Clause 52.232-18 - Availability of Funds is in effect for this notice of intent. As prescribed in 32.705-1(a), funds are presently not available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made.
