- This is a SOURCES SOUGHT NOTICE only. Responses to this notice will be used for information and planning purposes. No proposals are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The VA Palo Alto Health Care System is seeking sources to provide the following items for use with the daVinci surgical robot system: 5MM Disposable Accessories 5MM Endowrist Instruments 5MM Reusable Accessories Vision System/Accessories. 8MM Reusable Accessories Seals Offerors may identify their interest and capability by submitting literature of their proposed product. Offerors must identify their product as "brand name or equal . The œequal product must have the same physical, functional, or performance characteristics as the items reference above. The œequal product must be compatible with the daVinci surgical robot system. Offerors response shall include the Offerors Company Name, DUNS number, address, point of contact, title, phone number and email address. Offeror shall also identify their type of business (Small or large) and socioeconomic status whether Service Disabled Veteran Owned, Veteran Owned, Hubzone, 8(a), Woman owned, Small Disadvantaged, etc. The NAICs code for this is 39112, Surgical and Medical Instrument Manufacturing (size standard of 500). Responses are to be sent electronically to carol.lam@va.gov. Only emailed requests received directly from the offeror are acceptable. Facsimile responses will not be accepted. After review of the responses to the sources sought notice, a solicitation announcement may be published on the FBO website. Responses to the sources sought notice are not considered adequate responses to the solicitation announcement. All interested parties will have to respond to the solicitation announcement in addition to responding to the sources sought notice. Interested parties are reminded to register with Central Contractor Registration (CCR) at http://www.ccr.gov in order to be eligible for award of Government contracts.
