Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation VA258-12-Q-0742 is issued as a request for quotation. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular2005-36. (iv) This procurement is being issued as a 100% Small Business set-aside under NAICS code 562998 with a small business size standard of $7M. (v) CLIN 0001, (1) JB. (vi) The Contractor shall furnish all management, supervision, labor, transportation, materials and all other required items necessary to perform all operations in connection with the project titled "Grease Trap Cleaning Services" at the Southern Arizona Veterans Affairs Health Care System (SAVAHCS) campus, Tucson, Arizona. Work will consist of: Statement of Work - SAVAHCS has a continuous requirement to provide monthly pumping and cleaning of the main kitchen underground grease collection tank. The contractor shall provide all labor, material, equipment and supervision necessary to pump and clean the three chamber grease collection tank at Bldg 3 the main kitchen. Contractor shall perform these commercial services IAW with all federal, state, county, local, and VA requirements. (vii) Period of Performance: There shall be a base contract period with an additional four option years available for continuation of services. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated on price. (x) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (xi) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
