- Statement of Work JTFNMEALSEP001 Combined Synopsis / Solicitation JTFNMEALSEP001 Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. JTFNMEALSEP001 Solicitation document and all incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-60. Set Aside: 100% small business NAICS: 722320 Size Standard: $7,000,000.00 52.212-1 Instruction to Offerors Commercial Items - applies to this acquisition. The following addenda is provided to this provision: Request for Quotation should be submitted and shall contain the following information: Solicitation Number; Time Specified for receipt of offers; Name; Address; Telephone Number of Offeror; Terms of the expressed warranty; Price, Payment terms, Any discount terms, F.O.B. destination, CAGE number, DUNS number, Tax ID number and acknowledgement of all solicitation amendments. Quotes must indicate quantity, unit price and total amount. Offerors that fail to furnish the required information, or reject the terms and conditions of the solicitation may be excluded from consideration. 52.212-2 Evaluation -- Commercial Items: Price Statement of Work for Catered Meals JTFNMEALSEP001 Period of Performance: 29 Sep - 19 Nov 2012 Line Item 1: Contractor shall cater up to 1882 breakfast meals for up to 40 personnel, to a designated location in the vicinity of Nogales, AZ. Breakfast meals shall be served daily, Monday thru Sunday, starting at 0600 thru 0630. Breakfast meals shall offer a variety of items that shall include fruits, juice, cold cereal, bacon/sausage/ham, eggs, toast, pancakes/waffles and drinks such as milk, coffee and water. Line Item 2: Contractor shall cater up to 1882 dinner meals for up to 40 personnel, to a designated location in the vicinity of Nogales, AZ. Dinner meals shall be catered daily, Monday thru Sunday, starting at 1830 thru 1930. Dinner meals shall consist of servings that shall include the basic food groups; grains, fruits, vegetables, meats, and dairy product along with a desert and dinner drink. Requirements/Evaluations Factors for Catered Meals: a. Contractor shall be responsible for providing plates, eating utensils, drinking glasses/cups, and napkins and associated condiments for all meals. b. Contractor shall provide sufficient quantities of ice necessary for storage, serving and for cold drinks for meals. c. Contractor shall ensure that enough contractor personnel/staff are present to accommodate adequate service for each meal given the total number of meals required. Meals may be served buffet and/or full service style. d. Cost of catered meals shall include all costs associated with transporting, preparation, and serving the meals. e. The contractor shall provide, for the duration of the event, a seven (7) day menu, at a minimum one week prior of the scheduled contracted meals, to the managing and/or supervising representative for review and approval. f. Contractor shall provide breakfast and dinner meals seven (7) days a week. g. Contractor shall provide breakfast and dinner meals within five (5) miles of contracted lodging. h. Contractor shall provide prepared meals in a certified kitchen and shall possess the appropriate state, county and city licenses and permits to serve food. i. Contractor shall be responsible for disposal of all waste created by serving contracted catered meals.
