PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for VA248-12-R0607A, issued as a Request for Proposal (RFP), quotes are being requested and a written solicitation will not be issued. This notice and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-58 18 May 2012. The North American Industry Classification Systems (NAICS) code is 424430, and size standard is 100 Employees. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE AND TELEPHONE REQUESTS FOR COPIES WILL NOT BE HONORED. GENERAL: The contractor shall provide milk and dairy products to the James A. Haley VA Medical Center (hereafter referred to as the JAHVA) located at 13000 Bruce B. Downs Blvd, Tampa Florida 33612. The VA intends to award a firm, fixed price contract for the provision of milk and dairy products. The quantities listed in the attached Pricing Schedule are estimated quantities for the Nutrition & Food Services. Contract will be for a base and four option years estimated to begin September 1, 2012 DESCRIPTION/SPECIFICAT1ONS/WORK STATEMENT Delivery of Milk and Dairy Products Specifications for Milk and Dairy Products James A. Haley VA Medical Center SCOPE OF WORK: A. General: The scope of this requirement involves providing milk and dairy products, which conform to the specifications outlined and referenced herein, to the JAHVA's Nutrition & Food Service. B. Quantities: The quantities listed in the attached Pricing List represent the estimated quantities that will be ordered over a 12-month period. The actual quantity ordered may vary from this estimate but shall not exceed the total estimated quantity for the 12-month period without the prior written approval of the Contracting Officer. C. Period of Performance: The period of performance for the provision of milk and dairy products shall be a 11-month base year period from 01 September 2012 - 31 August 2013 plus four (4) 12 month option years D. Ordering procedures: Estimated milk orders will be submitted three times each week (Friday, Tuesday, and Thursday by 3:00 p.m.) with 24 hours notice. Quantities ordered may vary from order to order. The Contracting Officer or his/her designee will place orders. Telephone, oral, and wire transmission orders are authorized. E. Deliveries and Invoicing: Deliveries shall occur twice per week between the hours of 6:30 a.m. and 9:00 a.m. on Monday, Wednesday and Friday. Requests for deliveries on different days and/or at a different time shall be submitted to and approved by the Contracting Officer and his/her designee in writing. ATTACHMENT 1 FULL RFP ATTACHMENT 2 FINAL PRODUCT LIST FOR SON. INVOICING - Submitted monthly in the arrears to the VA Payment Center Austin TX. ****RFP ELECTRONIC SUBMISSIONS: ALL interested parties may submit a proposal which will be considered by this agency. All interested parties must provide the submissions by the RFP due date: 16 August 2012 in response to this RFP solicitation. Those who fail to complete and submit the requirements may be considered non-responsive. LATE SUBMISSIONS WILL NOT BE ACCEPTED. ****TYPE of CONTRACT: Firm Fixed Price ****PERIOD of PERFORMANCE: One year from contract start with 4 option years ****DELIVERY AND ACCEPTANCE: FOB Destination ****Estimated Start Date: September 1, 2012 ****RFP QUESTIONS: All inquiries must be via email, and all questions and responses will be posted to the FEDBIZOPS website as an amendment to this original announcement. Questions concerning this solicitation MUST be submitted via email no later than August 14, 2012, 9:00 am Eastern Standard Time (EST). ****RFQ DUE DATE: Responses to this RFP, shall be via electronic submission to trina.berry@va.gov on or before August 16, 2012 at 5:00 pm EST.
