Attachment 4: SF-18 Request for Quotation Attachment 3: Prospective Respondents Packet Attachment 2b: Diagram Attachment 2a: Statement of Work Attachment 1: DOL Wage Rates for Story County, IA This construction solicitation is to seal cracks and repair a section of an existing asphalt parking lot at the USDA ARS Plant Introduction Station in Ames, IA. The work will consist of sealing approximately 550 linear feet of cracks with hot pour rubber and repairing an approximate 144 square foot section of the asphalt lot. AGAR 452.211-73 Attachment to Specifications/Statement of Work The attachments are hereby made part of this solicitation and any resultant contract. Attachment 1: Department Of Labor Wage Determination for Story County, IA (10 pages), Attachment 2: Statement Of Work with diagram (2 pages), Attachment 3: Prospective Respondents Packet (48 pages), and Attachment 4: SF 18 Request For Quotation (1 page; blocks 8, 11e-f, 12-16). Price Range - Less Than $25,000 Site Showings will take place at the Plant Introduction Station, 1305 State Ave., Ames, IA 50014 and may be coordinated with this project's technical point of contact: Larry Lockhart (515-294-3359). 52.228-13 ALTERNATE PAYMENT PROTECTIONS (JUL 2000) The Contractor shall submit one of the following payment protections: Irrevocable Letter of Credit (ILC) AND a: Payment Bond, Certificate of Deposit, Certified Check, or Money Order. The amount of the payment protection shall be 100 percent of the contract price. The submission of the payment protection is required within 7 days of contract award. Please furnish Quotations to the Issuing Office at USDA-ARS-NADC, 1920 Dayton Ave., Building 24, Ames, IA 50010 on or before Close of Business 09/10/2012 at 9:00am Central Time. Funds are currently available for this contract. Questions, technical literature, and other documents may be addressed to the Purchasing Agent, Matthew Treska, via email at matthew.treska@ars.usda.gov. Please reference the solicitation number on all documents. AGAR 452.211-74 Period of Performance- The performance time is 60 days after issuance of Notice To Proceed (NTP). Anticipated Award and Notice to Proceed is September 17, 2012. Anticipated end date is November 16, 2012. See FAR 14.214-7 "Late Submissions, Modifications, and Withdrawals of Bids," for treatment of late bids/quotes. Each offer is to be submitted according to the instructions. FACSMILE/EMAIL BIDS WILL BE ACCEPTED. Contractors may submit facsimile quotes as response to this solicitation IAW FAR Part 52.214-31 FACSIMILE BIDS (DEC 1989). However, quotation must arrive at the place and by the time, specified in the solicitation. The Government will not be responsible for any failure attributable to the transmission or receipt of the facsimile quote. It is the responsibility of the Offeror, prior to the offer due date and time, to verify successful transmission in accordance with the Proposal Response (Submission) instructions. The FOB Point, inspection location, and acceptance location shall be Destination. Offerors are advised that award for this solicitation shall be made in the aggregate (all or none). ADDITIONAL INFORMATION: Be advised that all interested parties must be registered in the Central Contractor Registration (CCR) Database within the Single Award Management (SAM) program in order to receive an award. If you are not registered you may register at www.sam.gov. The government will evaluate quotes resulting from this RFQ based upon best value to the Government. The responsible offer conforming to this solicitation will be the most advantageous to the Government based upon the following best value criteria: - Lowest reasonable price - Responsible, responsive contractor
