Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA244-12-R-0710 and is issued as a Request for Proposal (RFP).The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-58. The associated North American Industrial Classification System (NAICS) code for this procurement is 541990 with a small business size standard of $7.00M.This requirement is a Small Business set-aside and only qualified offerors may submit bids. The solicitation will start on the date of posting and the solicitation will close on September 4, 2012 3:30PM Eastern Time. The VHA NCO 4 Station 540 - Louis A. Johnson VA Healthcare System in Clarksburg, WV requires Health Physicist Services. The Contracting Officer intents to award a firm fixed price contract for a base period of October 1, 2012 through September 30, 2013 with four 1 year option periods to be exercised at the discretion of the government. The Medical Center has a requirement for a board certified health physicist to provide quarterly equipment testing, annual radiation safety training and HAZMAT training. The Contractor will provide annual medical physicist reports for the equipment listed in the RFP. Reports must contain, at a minimum, the information listed with each type of equipment. The Contractor will review all pertinent records to verify compliance with the Nuclear Regulatory Commission (NRC), National Health Physics Program (NHPP), Joint Commission (JC), and state requirements on a quarterly basis. The Contractor shall prepare all license applications and amendments, including interpretation of and responsive actions to regulatory agencies. The Contractor shall attend quarterly Radiation Safety Committee meetings held at the VA. The Contractor shall maintain and support the VA's As Low As Reasonably Achievable (ALARA) program. The Contractor will assume the duties of Radiation Safety Officer for the facility. The Contractor will ensure all aspects of the I-131 Thyroid Quality Management Program are within compliance standards. Compliance standards will be reviewed quarterly and revised as requirements dictate. The Contractor will provide a review summary of the radiation safety program for the preceding twelve (12) months on a quarterly basis. The Contractor will provide film and ring badges to support radiology staff, nuclear medicine staff, and all others deemed necessary by the Radiation Safety Committee. The Contractor will review and sign quarterly dosimetry reports generated by the dosimetry vendor. The Contractor will identify those staff who have exceeded ALARA I or ALARA II and recommend ways to reduce radiation exposure to those staff in the future. In addition, the Contractor shall provide individual, annual dosimetry reports, for any badged VA employee or VA contractor, to the Contracting Officer's Technical Representative (COTR) no later than 90 days after the close of the calendar year. The Contractor will be responsible for reviewing the Nuclear Medicine policy and procedure manual to ensure all policies/procedures are up to date and in compliance with current standards. The Contractor shall sign the first sheet of the manual attesting to its completeness annually or at any time policy changes. The Contractor will provide review of imaging policies and procedures encompassing other modalities when necessary. The Contractor, in his or her capacity as the RSO, will be available by telephone and/or e-mail, on a continuous basis, for consultation when questions or problems arise. More specific details are available in the RFP. Offeror must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. The selected Offeror must comply with the following commercial terms and conditions, in the attached RFP.
