Therefore; bidders must submit a bid price for the Base Year and each Option Year. Failure to submit a price for each year may cause the bid to be found non-responsive and rejected. DESCRIPTION OF SERVICES/STATEMENT OF WORK 1. GENERAL: The Contractor shall provide all materials, labor, and supplies necessary to properly perform all dental laboratory procedures requested in accordance with the written dental laboratory prescriptions from bldg. 23, first floor Dental Service Providers at the Bay Pines VA Healthcare System, 10000 Bay Pines Blvd., Bay Pines, Florida 33744. 2. SCOPE: The contractor shall provide transportation for pick-up and delivery of all dental cases from the Dental Lab at the Bay Pines VA Healthcare System on a daily basis, and complete all phases of dental prosthetic procedures in a timely manner. 3. TYPE OF CONTRACT: The Department of Veterans Affairs (VA) intends to issue a firm fixed-price requirements contract to provide commercial services to implement. The selected Contractor will have a favorable record of past performance in providing the services described in the Statement of Work (SOW), and be familiar with the legal requirements related to the services. 4. TERM OF CONTRACT: This contract is effective from October 2012 through September 2013. The contract is subject to the availability of VA funds. 5. SERVICES TO BE PERFORMED: The contractor will provide daily pick-up of all lab cases from Dental Service, Bldg. 23, VA Medical Center, Bay Pines, FL during the operating hours of 8:00 am through 3:00 pm, Monday through Friday, except for federal holidays (see "Work Hours"). The COTR will be notified a day in advance of actual pick-up. Vendor will perform all operations in a prudent, conscientious, safe and professional manner and will comply with all safety requirements set forth in applicable State, Federal and Local laws and regulations. Vendor must be located within 15 miles of the Bay Pines VAHCS Dental Clinic. Vendor must produce all product work within the borders of the United States. Vendor must use ADA approved materials. Vendor must follow the Dental Practice Act of FL. Vendor will provide the necessary personnel and materials to complete all phases of dental prosthetic procedures, to include, processing complete and partial dentures, pouring/pinning models, making custom trays, wax rims, set-ups, casting framework, crowns and bridges, repairs, partials, flexible, (Dura Flex) diagnostic wax-ups, custom welding, orthotic devices, and custom gold and ceramic work.
