Description
COMBINED SYNOPSIS/SOLICITATION PROVIDE ALL TOOLS MATERIALS AND LABOR FOR THE INSTALLATION OF A DUAL BELT TREADMILL WITH INTEGRATED FORCE PLATES BRAND NAME OR EQUAL TO BERTEC FULLY INSTRUMENTED TREADMILL (FIT). (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All Small Businesses submitting an offer must be registered on www.sam.gov and https://orca.bpn.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number VA246-12-Q-2652. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-57. (IV) This procurement is restricted to Small Business Concerns. The associated NAICS code is 339112. The small business size standard is 500 employees. (V) This combined solicitation/synopsis is for purchase of the following commercial items: Instrumented Dual belt treadmill with two integrated force plates. 1 EA Treadmill AM6500 Amplifier with USB output with data acquisition software. 1 EA AM6501 Analog amplifier with fixed gain (+/- 5V output) 2 EA FPA-10 Force plate to amplifier cable (10m) 3 EA AMO-BNC-01 Amplifier output cable with BNC connector for each channel 2 EA TM08 Mounting plate and mounting hardware Mounting 1 EA Plate Incline Feature Treadmill incline system. 1 EA Installation and Staff Training 1 EA (VI) Description of requirements is as follows: Salient Characteristics: The FIT will contain a modular incline feature for uphill and downhill analysis of movements. The load range is 1,100 pounds vertical and 550 pound horizontal force capacity. The FIT has 15 degrees (27%)f incline and can also be run in reverse to use decline feature. The speed range is 0-15 miles per hour and independent control of belts in both forward and reverse. The unit contains software development kit to allow therapist to develop their own treadmill control protocols. Training/Inservice (Applications): -Availability of vendor to provide on-site training and in what timeframe. Maintenance: -Service manuals must be completed to include; theory of operation, troubleshooting, "complete" parts breakdown and all applicable schematics. Other: -Provide documentation of government facilities where this equipment is already in use. Identify references with points of contacts and telephone numbers. (VII) Required delivery 45 days ARO. Place of delivery is Richmond VA Medical Center, 1201 Broad Rock Blvd, Richmond, VA 23249. Delivery shall be FOB Destination to Richmond, Virginia. (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jun, 2008), applies to this acquisition. (IX) Evaluation - The Government will award a firm fixed priced contract to the responsible offeror whose offer, conforming to the requirements in this request for quote, will be most advantageous to the Government, price and other factors considered. A best value award will be made based upon the following factors: Technical and Price. a. Technical: Ability to provide products that conform to the salient characteristics and requirements listed in this solicitation b. Price (X) The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (April 2011), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at https://www.sam.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. (XI) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items (Jun, 2010), applies to this acquisition. (XII) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Apr, 2011) applies to this acquisition. Questions must be received no later than 12:00 PM, August 14, 2012. (XV) Quotes are required to be received in the contracting office no later than 8:00 AM EST on July 13, 2012.
