- Combined Synopsis/Solicitation-Services This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This procurement is not a small business set-aside. Therefore, any responsible source may submit an offer. This announcement constitutes the only solicitation. Therefore, quotes are being requested and a separate written solicitation will not be issued. Solicitation number M67400-12-T-0148 applies and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-56 and Defense Acquisition Circular 91-13. The FSC code is J058 and the NAICS code is 334220. In accordance with the attached Statement of Work (SOW), the Government is soliciting quotes for the services for SWE-DISH repair All services, as annotated in the attached Statement of Work (SOW), awarded as a result of this RFQ are expected to commence no later than the date specified in the contract award and the SOW. The Government intends to issue a single award to the responsible offeror whose quote is the most advantageous to the Government considering Technical Capability, Price. Award will be made in aggregate. Therefore, any offer received without all services priced, as annotated in the attached SOW, will be excluded from competition. It is the offeror ™s responsibility to furnish sufficient information to the Government in order to determine acceptability of the offered services. Therefore, the offeror shall submit all technical information pertaining to their offered services in order for the Government to determine technical acceptability. The quotation, submitted in response to this solicitation, shall be formatted as follows, in separately bound volumes. Volume I - Price Quotation CLIN0001 Evaluation and testing CLIN0002 Replacement Modem if necessary CLIN0003 Freight Volume II - Part I - Technical Capability STATEMENT OF WORK (SOW) SWE-DISH Satellite System Repair. 1. GENERAL. The services are required to complete maintenance on the Swe-Dish Sattelite Dish. The Swe-Dish Sattelite Dish is required to support the Commanding General of III MEF with communications services 2. OBJECTIVE. The objective is to have the contractor complete the maintenance that is beyond this command ™s capability. 3. SCOPE OF WORK: The contractor shall provide all qualified personnel, supervision, supplies, materials, parts, components, equipment, tools, and transportation to perform preventative maintenance and repair services for Secure Telephone Equipment telephones. 4. SPECIFIC TASKS. The following maintenance issues for each serial number will be completed: 1.40099 / A707470: Evaluation of Equipment to determine repair, replace rotation plate w/antenna quick release, azimuth motor, DC/DC converter kit tested IPT, and DC output Cable IPT. 5. REPORTING REQUIREMENTS. a. Email correspondence is required by this Government Representative 6.DAMAGE TO GOVERNMENT PROPERTY In all instances where government property or equipment is damaged by the contractor, a full report of the facts and the extent of the damages shall be submitted to the government representative within 24 hours following the incident. The government representative shall forward the report to the contracting officer within 24 hours of receipt. If the contractor is found at fault, the contractor shall be held responsible and will reimburse the government for all damages. SECTION II: DEFINITIONS 2.0 Contracting Officer (CO): U.S. Government official authorized to enter into, make changes to, or terminate contracts on behalf of the US Government. Contracting Officers are responsible for ensuring performance of all necessary actions for effective contracting, ensuring compliance with the terms of the contract, and safeguarding interests of the United States in its contractual relationships. 2.1 Government Representative (Gov ™t Rep): Government liaison, which functions as the technical representative monitoring, delivery and performance of the contract. Gov ™t rep is not authorized to enter into any agreements, obligate the government or direct any changes to the contract. Gov ™t reps (alternative gov ™t reps) of this contract are also responsible for escorting the contractor to the work site when needed and securing keys of the buildings during the periods of performance.
