- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation A12PS02238 is being issued as an RFQ. The solicitation document and incorporated provisions and clauses are those that are in effect through Federal Acquisition Circular (FAC) 05-60. Description of the requirements: Line Item #1: Boiler inspections - please request the statement of work (SOW) and the geographic locations to be sent to you via email (if not attached to this posting) by emailing the contract officer listed below. Other terms and conditions: The vendor must be responsive to the SOW and to the requirements listed in this solicitation for quotes. Along with the SOW the vendor must submit a chart for milestones when the boilers will be inspected (once inspections begin they must be finished within 30 days), once the purchase order is awarded inspections must begin with 60 days (unless modified). Milestones will be used in part to determine a payment schedule and to determine the progress of the vendor once work begins. The vendor must also submit information, if the purchase order is awarded to them, so a background check can be performed because some of the locations are located in schools. If there are any questions about the specifics on the SOW, please contact the COTR listed on the SOW. Once a contract is awarded please direct all questions to the COTR and CO. Date(s) and place(s) of delivery and acceptance and FOB point: The locations will vary, please contact the CO via email to obtain a SOW and a list of geographic locations. Once the purchase order is awarded work must begin within 60 days of award (this could be modified later) and work must be completed 30 days post initiation. Conflict of Interest (COI): All quotes, proposals, or bids must disclose any conflict of interest with any known person(s) working for the program office (requesting agency) or contracting office that is connected with this procurement, has authority to influence contracting decision, or has any appearance of a potential COI. By responding to this request for quotes (RFQ) the vendor is making an affirmative statement that they can or have the following capabilities: adequate financial resources to perform the contract; able to deliver the items within the specified time limits; have a satisfactory performance record for these items or services; have the necessary organization and experience or the ability to obtain them to successfully perform the proposed purchase order; be otherwise qualified and eligible to receive an award under the applicable laws and regulations. Proof of all these items may be required and the vendor must submit evidence if asked to do so. Determination of responsibility shall always be an evaluation factor. All quotes are due on: 9/5/2012, by 3 P.M. Eastern time and shall be submitted by email (no faxes are allowed) to the contract specialist's email, ricky.mills@bia.gov. All questions MUST be asked by close of the business day via email (ricky.mills@bia.gov) 5 day(s) prior to solicitation closing. The Government will award a contract resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate offers: price, technical capability, and past performance. Acceptance will occur once the vendor acts upon the purchase order, signs the purchase order, or gives written email of acceptance after a purchase order is transmitted.
