- Attachment B - Past Performance Survey Attachment A - Non-Disclosure Agreement 12-233-SOL-00414 Percussion Support & Training (Amendment 0001) PLEASE NOTE ANY ADDITIONAL REQUESTS TO EXTEND THE PROPOSAL SUBMISSION DATE WILL NOT BE CONSIDERED. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 12-233-SOL-00414 and it is issued as request for proposal (RFP). Please view attachments This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60. This is a total small business set aside and the associated NAICS code is 511210 and small business size standard is $25M The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items is applicable. The Government will award a contract resulting from this synopsis/solicitation to the responsible offeror that meets the evaluation criteria. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. Please note: An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://sam.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. Solicitation Technical Inquiries and Questions All questions regarding the solicitation, of a contractual nature or technical nature, must be submitted in writing to Charles R. Hicks, Contract Specialist, via email at Charles.Hicks@psc.hhs.gov by 12:00 PM Eastern Time August 23, 2012. PSC will not consider any questions received after that date. Proposal Submission Offerors who are interested in this solicitation must provide the contracting specialist with their proposal NO LATER THAN 12:00 PM EASTERN TIME AUGUST 30, 2012 VIA electronic copy to Charles.Hicks@psc.hhs.gov with the solicitation number and title in the subject line. A hard copy and CD must also be submitted by the closed date above. The hard copy and CD submission must both arrive by the closing date specified above in order to be considered. The hard copy and CD must clearly state the solicitation number, contractor name and technical and business proposals MUST be submitted on separate disks. The Offeror shall submit a hard copy and CD for the Technical Proposal and a hard copy and CD for the Business Proposal. Failure to separate the technical and business proposals will delay the review process. Late submissions, modifications, and withdrawals of bids will be process in accordance with FAR 52.212-1. If discussions are deemed necessary, the competitive range will be established and discussions will take place. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. If the Government is not in receipt of an electronic copy via email, hard copy and CD the Offerors proposal by the designated due date indicated on the solicitation, the proposal may not be accepted.
