Wage Determination Appendix C Statement of Work This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested, and a separate written solicitation will not be issued. This solicitation will be solicited under Other Than Full and Open Competition. This is a Request for Quote (RFQ) No. F3QTBA2174AM01 THIS SOLICITATION WILL NOT RESULT IN AN ORDER AGAINST AN FEDERAL SUPPLY SCHEDULE. ANY OFFER WHICH IS PREDICATED UPON THE AWARD BEING PLACED AGAINST AN FSS WILL NOT BE CONSIDERED. SMALL BUSINESS SIZE STANDARD: (a) NAICS: 332311 (b) NR OF EMPLOYEES: 500 SERVICE REQUIREMENTS: CLIN Description Price 0001 Provide all labor, parts, and material necessary to dismantle and transport for relocation (within 15 miles) twenty-eight (28) existing Wenger Music Library Systems. Once relocated, system will be reinstalled in new location. IAW attached Statement of Work dated 23 July 2012. 0002 Provide all labor, parts, and material necessary to dismantle and transport for relocation (within 15 miles) six (6) Wenger Sound Modules. Once relocated, modules will be modified and re-installed in new location. Re-installation shall include all parts necessary to convert existing units from fan rooms to hook directly into HVAC system. Re-installation shall include new vertical and horizontal closures. IAW attached Statement of Work dated 23 July 2012. The Government will make all necessary utility connections to electrical and HVAC. Mfr: Wenger Corporation Justification for Sole Source: Wenger Corporation is the Original Equipment Manufacturer and therefore has proprietary data on design and manufacturing of component parts. Wenger also holds warranty as the OEM. Relocation of Library system requires tear down, set up, and all parts associated. Relocation of the Sound Modules requires modification of existing set up and all parts required. OEM is the sole provider of parts and designs. Price(s) should be FOB Destination (Robins AFB GA 31098). In addition to price and delivery, quote should include your cage code, DUNS number, and taxpayer ID number.
