- W912ES-12-T-0145 Performance Work Statement/SOW W912ES-12-T-0145 SF 1449 Solicitation/RFQ The U.S. Army Corps of Engineers, St. Paul District is soliciting quotes for the service of hazardous damaged tree cleanup and removal of stumps and filling holes left by the storm on 02 July 2012 at Lake Winnibigoshish Dam and Recreation Area, near Deer River, MN. All work shall be in accordance with the attached performance work statement and enclosed Dept. of Labor wage rates. F.O.B. destination is requested. Solicitation number W912ES-12-T-0145 is issued as a 100% Small Business (SB) set-aside in accordance with FAR Part 19. Item 0001 is informational: REQUEST TO CLEAN UP DAMAGE DONE AT WDRA DURING THE JULY 2, 2012 STORM. THE WORK SITE IS LOCATED AT THE U.S. ARMY CORPS OF ENGINEERS LAKE WINNIBIGOSHISH DAM AND RECREATION AREA, NEAR DEER RIVER, MN. ALL WORK IS TO BE PERFORMED IN STRICT ACCORDANCE WITH THE ATTACHED PERFORMANCE WORK STATEMENT SPECIFICATIONS AND DEPT. OF LABOR WAGE RATES. YOU MAY PLACE YOUR QUOTE GRAND TOTAL ON THIS LINE ITEM. Item Number 001AA is for Lake Winni Hazardous Tree Clean-up, Removal of Hazard Trees (85 each). Item Number 001AB is for Lake Winni Hazardous Tree Clean-up, Removal of Stumps and Filling Holes (25 each). Please see separate attachment for Performance Work Statement. The resultant contract will be a Firm-Fixed Price contract. The associated NAICS code is 561730 with a size standard of $7.0 million. Offeror must have a company account in the System for Award Management (SAM) database at website http://www.sam.gov prior to contract award. A determination by the government to not award based on the responses to this solicitation is solely within the discretion of the government. Email responses are the preferred method to submit a quote; however, quotes may also be faxed to Attention: Deborah Lawrence at 651-290-5706. Quotes may also be mailed to the U.S. Army Corps of Engineers, St. Paul District, Attn: Contracting Division, 180 5 th Street East, Suite 700, St. Paul, MN 55101-1678. All quotes must be inclusive of all labor, materials and transportation. No additional costs will be enumerated after award.
