Description
Oklalahoma Area IHS Federal Labs for Hematology requirement Hematology Testing Requirements and Equipment Hematology Solicitation for OCA Federal Labs This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, Streamlined Procedures for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is HEM-OCA-12-0001. The solicitation is issued as a Request for Quotes. Provisions and clauses in effect through Federal Acquisition Circular 05-60 are incorporated. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at: and https://www.acquisition.gov/. The NAICS code is 334516, with a small business size of 500 employees. This is a Full and Open Competition requirement; all qualified vendors are encouraged to submit a proposal. Offers are due by September 14, 2012, 4:00 PM, Central Standard Time. The point of contact is Sara Marcus, CS, at (405) 951-3998 or sara.marcus@ihs.gov. Offers shall be submitted by mail or courier (USPS, UPS, Fed Ex, etc.). Questions regarding the requirement must be submitted by e-mail to Sara Marcus no later than September 3, 2012, 4:00 PM Central Standard Time. No questions will be entertained after this date. Requirement: The Oklahoma City Area (OCA) Indian Health Service (IHS) Laboratories has a requirement for equipment, services and supplies required to perform and technologically standardize hematological testing services across OCA Laboratories while ensuring an uncompromising quality of patient care. Refer to the attached documents for full details. This is a "brand name or equal" solicitation. The brand name description of the equipment and products reflect Sysmex. The successful offeror shall provide all necessary equipment, installation and testing, preventive maintenance agreement on the equipment, training, 24/7 support and supplies required to conduct the necessary testing (see attached documents). It is the intent of the Government to establish a fixed price "Cost per Test" type contract as a result of this solicitation. EVALUATION FACTORS FOR AWARD (See Attachments for full details): The Government intends to make one award resulting from this solicitation to the responsible offeror whose offer is the lowest price technically acceptable offer. Proposals shall provide documentation that shows the proposed equipment, services and supplies/products that meet the salient requirements for the hematology testing requirement and verification of successful past performance. Evaluation of price will be based on the offeror's total price for all line items as delivered to the specified destination. The following factors shall be used to evaluate offers: 1. Technically acceptable in conformance to the requirements of the solicitation. 2. Low overall price. 3. Past performance. The offeror's initial proposal shall contain the offeror's best terms from a price and technical standpoint. The Government intends to evaluate offers and award a contract without discussions, but reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government reserves the right to make no award as a result of this solicitation. INSTRUCTIONS TO OFFERORS (See Attachments for full istructions): Offerers shall provide in their proposal: 1. Technical Proposal: Technical documentation (including but not limited to) product literature, equipment specifications, preventive maintenance agreement, supplies/products description/item numbers, etc, to allow for a thorough evaluation of the proposed equipment, services and supplies offered. 2. Completed Price Schedule (attached). Offerors must propose a monthly price for each facility for the base year and each option year. The base and each option year schedule and total price must be completed. Proposed price shall take into consideration estimated tests required, equipment, services and supplies required. 3. References. Include contact name, title, telephone number and email from 3 current similar size contracts. 4. Completed Representations & Certifications or statement indicating ORCA completion. 5. FSS schedule numbers and pricing, if applicable. 6. Active registration in the System for Award Management (SAM).
