Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation VA258-12-Q-0858 is issued as a request for quotation. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular2005-36. (iv) This procurement is being issued as a 100% Small Business set-aside under NAICS code 423450 with a small business size standard of $14M. Certified Service Disabled Veteran Owned and Veteran Owned Small Business concerns, will receive up to a 10% allowance for consideration of their submitted pricing between their offer and that of offers from Small Business concerns (v) Schedule of Pricing: CLIN 0001 - Covers the below requirement. Please provide your vendor offer on the below schedule. (vi) STATEMENT OF WORK for "PULMONARY FUNCTION TESTING AND PMA." Contractor shall provide all labor (certified technicians only), tools, travel costs and related expenses, supplies, required parts for and annual preventive maintenance service, materials, facilities, transportation, and all other necessary elements required to provide "Pulmonary Function Testing and PMA" for the below listed equipment at the Southern Arizona Veteran's Affair's Healthcare System (SAVAHCS), VA Campus, 3601 S. 6th Ave., Tucson AZ 85723. Requirements are as follows: Preventive maintenance on each of the following Medical Graphics Corporation (also known as MedGraphics) Pulmonary Function Test units: Platinum Elite DX Serial Number: 233000220 Platinum Elite DX Serial Number: 233000219 Elite DX Serial Number: 212000719 Units shall be serviced on site only. Vendor is required to sign in upon arrival to facility through Biomedical Engineering admin office located in Building #2, room S306 during business hours. Business hours are defined as M-F 0800-1630 not including federal holidays. A field service report will be provided by vendor within 48 hours after completion of work. (vii) Period of Performance: This service will be performed within 30-60 calendar days of contract award. Contractor shall submit recommendation for Contracting Officer consideration. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated on price. (x) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (xi) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. VAAR 852.203-70 - Commercial Advertising, VAAR 852.203-71 - Display of Department of Veteran Affairs Hotline Poster, VAAR 852.219-10, VAAR 852.219-11, VAAR 852.237-70 - Contractor Responsibilities, VAAR 852.273-76 - Electronic Invoice Submission. Department of Labor Wage Determination number 2005-2023, Revision #16, Dated 06/19/2012 is hereby incorporated into this synopsitation and any subsequent contract award.
