Description
RFQ - EE133F12RQ1155 Unpaid Delinquent Tax Liability (Attachment A) Brand Name or Equal Specifications The United States Department of Commerce (DOC), National Oceanic and Atmospheric Administration (NOAA), National Marine Fisheries Service (NMFS), Northeast Fisheries Science Center in Woods Hole, MA is soliciting quotes for the below mentioned Temperature Depth probes and associated components/software - Brand name or Equal. This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number EE133F12RQ1155, in accordance with guidelines of FAR Part 12, Commercial Items. This is a 100% small business set-aside. The North American Industry Classification System (NAICS) code for this requirement is 541990, with less than $7,000,000 in average annual receipts. The National Oceanic and Atmospheric Administration (NOAA) will award a single Firm Fixed Price type contract based on the Lowest Price Technically Acceptable. The following supplies are being procured as BRAND NAME OR EQUAL: Line # / Brand / Item Description / Quantity 0001 / Star Oddi / DST centi-Temperature Depth Probe / 25 EA 0002 / Star Oddi / Protective Housing / 25 EA 0003 / Star Oddi / TD Probe Communication Box / 3 EA 0004 / Star Oddi / Software Interface License / 3 EA 0005 / Shipping The delivery will be FOB Destination and the address zip code is 02543. Requested delivery is 30 days after award. Please provide the cost of freight charges on line 0005 if applicable. Please see attached documents for Brand Name or Equal Specifications and RFQ. Vendors are required to submit a signed Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction Under any Federal Law (Class Deviation) (MAR 2012) (attached) with their quote. Responsible sources shall provide the following: 1. Price quote which identifies the requested supplies and unit prices 2. Signed Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction Under Any Federal Law (Class Deviation) (March 2012) (Attached) 3. Remittance address, Tax Identification number, Duns Number and Cage Code GOVERNMENT WILL NOT ACCEPT INCOMPLETE PROPOSALS. Offers will be evaluated on the prices as offered. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer will not be technically acceptable, and therefore will not be considered. Once an offer has been determined to be technically acceptable, and in compliance with all directions in this solicitation, the Government will award to the offeror with the lowest evaluated price. In order to comply with the debt collection improvement act of 1966, all contractors must be registered in the System for Award Management (SAM) to be considered for an award of a federal contract.
