- Request for Proposal (FA8601-12-R-0062) Statement of Work (Dated 15 Jun 2012) Executive Order 13221 (Dated 31 July 2001) THIS IS OFFICIAL NOTIFICATION OF THE ISSUANCE OF A REQUEST FOR PROPOSAL (RFP) Solicitation number for this requirement is FA8601-12-R-0062. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60, 27 Aug 2012. North American Industry Classification System (NAICS) Code: 335999 Size Standard: 500 Employees The AFLCMC/PKO Operational Contracting Division, Wright-Patterson Air Force Base, Ohio intends to award a firm-fixed price contract for QTY: (77,413) Smart Power Strips. In accordance with the attached solicitation, which includes the Statement of Work (SOW): "Smart Power Strips." Attachments: 1. Request for proposal ( FA8601-12-R-0062) 2. Statement of work (Dated 15 Jun 2012) 3. Executive Order 13221(Dated 31 July 2001) IMPORTANT NOTICE TO CONTRACTORS: All prospective awardees are required to register at the System for Award Management (SAM) and to maintain active registration during the life of the contract. SAM can be accessed at https://www.sam.gov. Any award resulting from this solicitation will include the clause at DFARS 252.232-7006 Wide Area Workflow - Payment Instructions (Jun 2012). Wide Area Workflow- Receipt and Acceptance (WAWF-RA) is the DoD system of choice for implementing this statutory requirement. Use of the basic system is at no cost to the contractor. Contractors must complete vendor training, which is also available at no cost at http://www.wawftraining.com. Prior to submitting invoices in the production system, contractors must register for an account at http://wawf.eb.mil. In addition to registering in WAWF, the awardee must complete the electronic registration at https://www.sam.gov AND a completed hard copy of FAR 52.212-3, Offeror Representations and Certifications-Commercial Items found in the SF 1449. For 52.212-3, complete paragraph b if SAM records are on file; complete paragraphs c-o if SAM records are not on file. Failure to submit all required documentation/information may result in a proposal being ineligible for award. In submitting a proposal, Offerors are agreeing to the terms and conditions as written in the entire solicitation. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions in full text may be accessed via the internet at http://farsite.hill.af.mil.
