Description
PROJECT DESCRIPTION: The G.V. (Sonny) Montgomery VA Medical Center requires a Contractor to furnish all labor, equipment, supplies, parts, supervision, and management to provide Preventative Maintenance (PM) and Emergency Repair (ER) of Exterior Lamps and Signs in accordance with the specifications, terms and conditions of this contract for the Department of Veterans Affairs, G.V. (Sonny) Montgomery VA Medical Center, 1500 E. Woodrow Wilson Drive, Jackson, Mississippi for a period of one (1) year to commence on or about October 1, 2012 through September 30, 2013, with four (4) one year extension option. All work is to be performed in accordance with (IAW) federal, state and local laws and ordinances by State of Mississippi qualified licensed electricians. OBJECTIVES: -To ensure all equipment is maintained to operate in a correct and safe manner as intended and as designed by the Original Equipment Manufacturer (OEM); and to ensure that all items are in compliance with applicable local, state and federal regulations. -To ensure that Preventive Maintenance (PM) is completed on time and in accordance with the Department of Veterans Affairs' recommendations. -To ensure equipment downtime is kept to a minimum. GENERAL: The Contractor shall perform, or cause to be performed, that portion of the Repair and Maintenance work necessitated by normal wear and tear. The Contractor shall repair or replace (when necessary) any and all defective Lamps, Starters and Other Parts deemed required to bring a unit back to working conditions. The Contractor shall wash/clean all fixtures annually, in the last month of the contract period (September of each specific year). The Contractor must inform the Contracting Officer's Representative (COR) at least two (2) business days prior to the washing/cleaning of the fixtures. Service calls will be responded to promptly after telephonic notification. Normal response time for 'Routine Service Calls' shall be within three (3) business days, after receipt of notification. Emergency Response Time shall be no less than two (2) Business days from the time the call is placed. Service personnel will be available for emergency calls twenty-four (24) hours a day, seven (7) days a week. The Contractor shall complete a Service Ticket after completion of any service or repair call and shall leave the Service Ticket with the Contracting Officer's Representative (COR). REPORTS: Upon completion of any Preventative Maintenance and Emergency Repair work, and within ten (10) business days after such service, the Contractor shall provide a written report of the services rendered to the Contracting Officer's Representative (COR) detailing the services. Any unsafe conditions encountered during the Preventative Maintenance and/or Emergency Repair work shall be reported immediately to the Contracting Officer's Representative (COR) or designee, advising the Department of Veterans Affairs of the unsafe conditions. IDENTIFICATION: Contractor's employees must have visible on their uniform, their name and company's logo. CERTIFICATION OF CONTRACTORS: Contractors shall be certified to perform Electrical work/services. The Contractor shall furnish a copy of up-to-date and current State certification (showing categories) to the Department of Purchasing and Contracting and to the COTR upon request and prior to the start of the contract. CONFORMITY TO REGULATIONS: The Contractor shall conform to Federal, State and local regulations governing, Exterior Lamps and Signs Maintenance Service. INTERFERENCE TO NORMAL FUNCTIONS: The Contractor may be required to interrupt its work at any time so as not to interfere with the normal functioning of the facilities, fire protection systems, facility patients' personnel, equipment and carts. HOURS OF OPERATION: The normal hours of operation for the facilities are 7:00 a.m. to 5:30 p.m., Monday through Friday. SCHEDULE OF VISITS: The Contractor shall provide a schedule of when Services shall be scheduled to the Contracting Officer Representative (COR) within fifteen (15) calendar days prior to the anticipated inspection date. Services shall be scheduled so as to be performed on definite appointed days and in no event shall the Contractor perform inspections outside regular agreed upon work hours without prior approval from the Contracting Officer Representative (COR). In the event climate or weather conditions make it unsuitable for Services, the entire service date will be re-scheduled to a date and time satisfactory to the Contractor and the Contracting Officer Representative (COR). Postponement will not reflect any penalties assessed to the Contractor or to the Government. POINT OF CONTACT (POC): The Contracting Officer shall delegate the Contracting Officer Representative (COR) to monitor the Contractor's performance. SERVICE VISIST/CALLS SIGN-IN: The Contractor will sign in and out when performing any service. The Contracting Officer Representative (COR) shall provide instructions as to the Point of Contacts and locations of sign-in/out. All service visits/calls shall be documented. The Contracting Officer Representative (COR) shall brief the Contractor on the preparation of this report. The Contractor shall be responsible for supplying, completing and submitting reports as required and requested by Federal, State or local ordinances which pertain to any duties contained in the contract. BASIS OF AWARD: The Department of Veterans Affairs intends on awarding a contract to a qualified firm that can meet the contract requirements and is the best overall value to the Government, price and other factors considered. CONTRACTOR SHALL SUBMIT ADDITIONAL INFORMATION AS REQUIRED IN SECTION 52.
