- PROJECT DESCRIPTION: The G.V. (Sonny) Montgomery VA Medical Center requires a Contractor to furnish all labor, equipment, supplies, parts, supervision, and management to provide Testing and Inspection of Water Well in accordance with the specifications, terms and conditions of this contract for the Department of Veterans Affairs, G.V. (Sonny) Montgomery VA Medical Center, 1500 E. Woodrow Wilson Drive, Jackson, Mississippi for a period of one (1) year to commence on or about October 1, 2012 through September 30, 2013, with one (1) one year extension option. All work is to be performed in accordance with (IAW) federal, state and local laws and ordinances and as required by the State of Mississippi Department of Health. GENERAL: Services will include, but will not be limited to: "Inspections three (3) times per week "Changing out chlorine bottles as needed "Maintaining well log "Performing all required sampling, ensuring the samples are received by the Mississippi State Department of Health, and providing the results to the Contracting Officer's Representative (COR) "Performing an annual inspection and providing the results to the Contracting Officer's Representative (COR) "Performing an annual pump test and providing the results to the Contracting Officer's Representative (COR) "Performing an annual back flow preventer inspections and providing the results to the Contracting Officer's Representative (COR) REPORTS: Upon completion of any service/work, and within ten (10) business days after such service, the Contractor shall provide a written report of the services rendered to the Contracting Officer's Representative (COR) detailing the services. Any unsafe conditions encountered during the service visit/call must be reported immediately to the Contracting Officer's Representative (COR) or designee, advising the Department of Veterans Affairs of the unsafe conditions. IDENTIFICATION: Contractor's employees must have visible on their uniform, their name and company's logo. CERTIFICATION OF CONTRACTORS: Contractors shall be certified to perform Servicing, Inspection, and Testing of Water Wells in the State of Mississippi, by the State of Mississippi Department of Health. The Contractor shall furnish a copy of its' up-to-date and current State certification (showing categories) to the Department of Purchasing and Contracting and to the Contracting Officer's Representative (COR) upon request and prior to the start of the contract. CONFORMITY TO REGULATIONS: The Contractor shall conform to Federal, State and local regulations governing, Testing and Servicing of Water Wells. HOURS OF OPERATION: Normal business hours are 8:00 a.m. to 4:30 p.m., Monday through Friday, excluding the holidays observed by the Federal Government, i.e., and any other day specifically declared by the President of the United States to be a national holiday. HOLIDAYS: The following federal holidays are observed: New Year's DayMemorial DayColumbus Day Thanksgiving DayMartin Luther King Jr.'s Birthday Independence DayVeterans DayChristmas Day President's DayLabor Day When a holiday falls on a Sunday, the following Monday will be observed as a legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed by U.S. Government agencies. SCHEDULE OF VISITS: The Contractor shall provide a schedule of when Services shall be scheduled to the Contracting Officer Representative (COR) within five (5) calendar days prior to the anticipated service date. Services shall be scheduled so as to be performed on definite appointed days and in no event shall the Contractor perform inspections outside regular agreed upon work hours without prior approval from the Contracting Officer Representative (COR). In the event climate or weather conditions make it unsuitable for Services, the entire service date will be re-scheduled to a date and time satisfactory to the Contractor and the Contracting Officer Representative (COR). Postponement will not reflect any penalties assessed to the Contractor or to the Government. POINT OF CONTACT (POC): The Contracting Officer shall delegate the Contracting Officer Representative (COR) to monitor the Contractor's performance. CENTRAL CONTRACTOR REGISTRATION (CCR): Federal Acquisition Regulations require that federal Contractors register in the Central Contractor Registration (CCR) database at http://www.ccr.gov and enter all mandatory information into the system. Award cannot be made until the Contractor has registered. Bidders are encouraged to ensure that they are registered in CCR prior to submitting their bid. VETBIZ.GOV VENDOR INFORMATION PAGES (VIP): This procurement is a total SDVOSB set-aside under Public Law 109-461. In order to be eligible for award, contractors must be registered in the VetBiz.gov Vendor Information Pages (VIP) at http://www.vetbiz.gov/. Award cannot be made unless the Contractor is registered. REPRESENTATIONS AND CERTIFICATIONS: Completion of Representations and Certifications in FAR 52.204-8 shall include completion of annual representations and certifications electronically via the Online Representations and Certifications Application (ORCA) website at http://orca.bpn.gov prior to submission of the bid. BASIS OF AWARD: The Department of Veterans Affairs intends on awarding a contract to a qualified firm that can meet the contract requirements and is the best overall value to the Government, price and other factors considered. CONTRACTOR SHALL SUBMIT ADDITIONAL INFORMATION AS REQUIRED IN SECTION 52. QUESTIONS: All questions regarding this solicitation must be submitted to the Contracting Officer in writing (email is acceptable).
