This is NOT a solicitation anouncemnt. This is a rquest for information only. The purpose of this souces sought synopsis is to gain knowledge of potential qualified sources and their size classifications(Service Disabled Veteran-Owned, Veteran-Owned Small Business, Hubzone, 8(a), small, small disadvantaged and large business) relative to NAICS code 541512 (size standard of $25.0) Responses to this request for information synopsis will be used by the government to make appropriate Acquisition decisions. Responseds to this souces sought shall NEITHR constitute responses to soliciatation which isnot currently available NOR a request to be added to a proisepctive biders list to receive a copy of the solicitation. The decision to solicit shall be solelt the Governments discretion. The Michael E. DeBakey VA Mecical Center (MEDVAMC) is seeking infomraiton on a potential contractor to provide Equipment, Software, Server License and Support from Health Care Control Systems Inc, being utilized in the Radiology Department. Contractor will be required to perform all support and services in accordance with the Statement of Work (SOW). If the Government decides to solicit for a contract for the aforementioned services, the Government contemplates an annual contract for a base term of 12 months with three one year option for renewal. Sources interested in this notice shall submit the following information: 1. Company name, address, point of contact, cage code, and size of business pursuant to North American industrial classification system (NAICS) 541512. 2. Evidences of applicable permits, licenses from Healthcare Control Systems Inc (HCS) to support OR Control System Software and licensing. 3. Evidences of capability to provide the aforementioned services. These evidences shall include but not limited to the list of existing customers having contracts for receiving the said services from contractor. 4. Responses to the following questions: (1) Is your firm registered in Central Contractor Registration: www.ccr.gov. (2) Is your business large or small business under NAICS 541512? (3) Is your firm a certified service-disabled veteran owned and veteran-owned small business? If yes, is your firm registered at http://www.vip.vetbiz.gov/general_user/search/default.asp and verified by the VA's Center for Veterans Enterprise (CVE)? (4) If small, is your firm qualified as a small, emergent business, or a small, disadvantaged business? (5) If disadvantaged, specify under which disadvantaged group and is your firm certified under section 8(a) of the small business act? (6) Is your firm a certified "hub zone" firm? (7) Is your firm a woman-owned or operated business? This is not a request for proposals or request for quotes and in no way obligates the government to award any contract. Responses to this notice shall be in writing by e-mail addressed to Corey Labbe @ corey.labbe2@va.gov. All responses must be received in writing no later than 12:00 PM, Central Standard Time on September 04, 2012. Telephone responses will NOT be accepted. Firms responding to this announcement, which fail to provide all of the required information, cannot be used to help the government make the acquisition decisions, which is the intent of this synopsis. This notice is to help the VA medical center in determining potential sources only. No solicitation is currently available. All firms responding to this request for information notice are advised that their response to this notice is not a request to be considered for a contract award or to be placed on the Government's prospective bidders list to receive a copy of the solicitation.
