- Floor Plan Buuilding 118, Floor Plan Building 49, Floor Plan Hanger 2, Floor Plan Statement of work This is a Total Small Business Set-Aside Request for Quote (RFQ) and the solicitation number for this requirement is F2V337229AUG2012. The 6th Contracting Squadron, MacDill AFB, Florida, requires the purchase and installation of locks as defined in the Statement of Work (SOW) see Attachment 1. CLIN DESCRIPTION QTY UNIT OF ISSUE UNIT PRICE TOTAL 0001 Purchase all required equipment, components, and installation of advanced proximity reader entry locks for 5 critical communications closets located in the following Buildings: Hangar 2, 1079, P-49, 347, and 1180 1 LumpSum All offers shall include pricing for the costs associated with installation and purchase of locks as described in the PWS. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58 effective 18 May 2012, DFAR DPN 20120420, and AFFAR AFAC 2012-0330. The North American Industry Classification System code (NAICS) is 561621, with a Size Standard of 500 personnel. A firm fixed price contract will be awarded. All interested parties must bid on all items. DELIVERY ADDRESS: MacDill AFB (Tampa), FL 33621 BASIS FOR AWARD: Award will be made to the lowest price offeror that is determined to be technically acceptable in accordance with the Statement of Work at Attachment 1. PROVISIONS/CLAUSES: The following Federal Acquisition Regulation (FAR), provisions and clauses apply to this solicitation and are incorporated by reference (provisions and clauses may be obtained via the internet: http://farsite.hill.af.mil, http://www.arnet.gov/far/, or http://safaq.hq.afmil/contracting) The following Air Force Federal Acquisition Regulation Supplement (AFFARS) clause applies and is incorporated in full texts: 5352.201-9101 Ombudsman. As prescribed in 5301-9103, insert the following clause: OMBUDSMAN (APR 2010) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (c) If resolution cannot be made by the contracting officer, concerned parties may contact the Center/MAJCOM ombudsmen Mr Gregory S. Concerns, issues, disagreements, and recommendations that cannot be resolved at the MAJCOM/DRU level, may be brought by the concerned party for further consideration to the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (703) 588-7004, facsimile number (703) 588-1067. (d) The ombudsman has no authority to render a decision that binds the agency. (e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer. Also your quote must list your DUNS number, CAGE code, and Federal TIN with Company name, POC and phone number, if you need to obtain or renew a DUNS number or CAGE code, please visit www.ccr.gov. Lack of registration in CCR database will make an offeror ineligible for award. A Site Visit will be held Monday, 10 Sept 2012 at 0900. The site visit will be limited to two representatives per company. No additional site visit will be conducted prior to award. Individuals wanting to attend the site visit will need to submit names to Genise Pennant, e-mail: genise.pennant@us.af.mil, no later than 7 Sep 2012 at 2:00 p.m. The following information is required for each attendee: As it appears on valid driver's license: Full name, full middle name (no nicknames) and social security number of the individual(s) attending the site visit and the respective company name, address and phone number. This information must be provided to ensure access to MacDill Air Force Base. All offerors are responsible for confirming receipt of e-mails to Ms. Pennant. Failure to submit required information by the due date may result in delay in processing base access pass, or denial of access to the base. In addition, to ensure timely arrival to the site visit, contractors attending will pick up base passes from the Visitor Reception Center located at 6801 S. Dale Mabry Hwy, Tampa, FL 33616 on 10 Sep 2012. Please meet at: 8011 Red Hibiscus Place Tampa Fl 33621 on MacDill AFB. Due to the multiple site locations, transportation will be provided. Please timely meet at the indicated location. The site visit will begin promptly at 0900 on 10 Sept. After the site visit, all vendor questions concerning this requirement must be submitted via e-mail no later than close of business (9:00 a.m. Local Time), 12 Sep 2012. Questions submitted after that time will not be answered prior to the response due date. No questions will be accepted via phone calls.
