Description
Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Request for Proposal (RFP) and the solicitation number is VA-244-12-R-1729. The government anticipates awarding a firm-fixed price contract resulting from this combined synopsis/solicitation. This solicitation is issued as a 100 % set aside for small business concerns. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov. STATEMENT OF WORK (SOW) UD Lobby Sculpture Project 1. Contract Title. Design, fabricate, install and project manage a sculptural installation that reflects the VAPHS Mission. Our Mission Statement The mission of VA Pittsburgh Healthcare System is to honor America's Veterans with world-class health care, train their future providers and advance medical knowledge through research. *Out of this statement we want to emphasize through this sculpture "to honor America's Veterans with world-class health care"* 2. Background. The advent of the new building at the University Drive Campus requires an art installation to finish off the main entrance - lobby area of that building. We need to visually display the VAPHS Mission. 3. Scope. This project entails providing a sculptural element that visually displays the VAPHS Mission and that represents all our Veterans. 4. Specific Tasks. The contractor shall design. The contractor will submit three design options, one of which will be chosen to be developed into the final installation. Through meetings with the contractor the chosen design will be developed into the final piece that will be installed in the lobby area. The contractor will fabricate the necessary elements to fully install the sculpture into the lobby of the University Drive Campus. The contractor will coordinate with the VA and install the sculpture in the lobby. This installation will most likely need to take place during off hours or weekends. 5.1 Task 1 - Enterprise Management Controls. 5.1.1 Subtask 1 - Design. Provide the VAPHS with three design that reflect the VAPHS Mission. One will be selected to be developed and realized as a sculptural installation. 5.1.2 Subtask 2 - Design Development. Meet with VAPHS for a total of 10 meetings to refine the sculpture 5.1.3 Subtask 3 - Fabrication. Upon completion of the design, fabricate the sculpture elements necessary for installation. 5.1.4 Subtask 4 - Installation. Upon completion of the fabrication, coordinate with VAPHS to install the sculpture in the lobby of the University Drive Campus. The installation will need to be during off hours and weekends so as not to interrupt the flow of patients at the facility. 5.2 Task 2. 5.2.1 Subtask 1 - Design. Present renderings or drawings of the three options of sculptures. 5.2.2 Subtask 2 - Design Development. Through a series of 10 meetings with the contractor refine design and develop the necessary drawings to fabricate the sculpture. 5.2.3 Subtask 3 - Fabrication. The contractor will fabricate all necessary parts and pieces to install the sculpture. 5.2.4 Subtask 4 - Installation. The contractor shall coordinate with VAPHS the installation of the sculpture during off hours and weekends. The contract shall supply all necessary materials and equipment to install the sculpture. Deliverables:See section 5.2 for deliverables. 6. Performance Monitoring The COR will work closely throughout the project and review each step with the contractor. 7. Security Requirements ISO/PO Comments: Contractor will be required to comply with physical security guidelines by either checking in with the VA Police each time they come on-site to perform contracted services or by obtaining a VA Contractor ID badge from the VA Police. Each Contractor employee working on-site must complete training course "VA Privacy and Information Security Awareness and Rules of Behavior" or each Contractor employee must be escorted at all times by a VA employee. Contractor may not have access to any VA sensitive information under this contract. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). No GFE or GFI will be provided to contractor. 9. Other Pertinent Information or Special Considerations. Vendor must have experience with the design, development, fabrication and project management of large scale installations in major public facilities, such as hospitals, airports, public parks, corporate headquarters and other large public buildings. There will be no follow on work to this contract and the contractor will not have access to any sensitive information. The contractor must ensure that all deliverables will arrive to the site in an undamaged condition. The COR(Contracting Officer Representative) is responsible for certifying that the work done under this contract is performed to time and standard. The COR will assure the inspection and acceptance of products provided incidental to services. 10. Risk Control There are no risks associated with this contract 11. Place of Performance. Work to be performed; 1.All design and fabrication is to be completed at contractor's sight. 2. The installation will be completed on the VAPHS site: University Dr. C 12. Period of Performance.
