Description
1. The Department of Veterans Affairs is seeking a business that is able to provide Cardiology services to the Central California Healthcare System (VACCHS), Fresno, CA. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60. Offers from all capable and responsible sources will be considered. Contractors responding to this notice need to provide the below information to eric.toliver@va.gov no later than September 24, 2012. 2. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60. The NAICS code is 622110, with a size standard of $34.5 million. 3. The contract period performance will be on or about October 1, 2012 to September 30, 2013 for a one-year base period, plus one (1) one-year option period. 4. Firms offering a response to this notice should ensure that they are registered in the System for Award Management (SAM) Database http://www.sam.gov/portal/public/sam/ and ensure that the NAICS code 622110 is included in the CCR profile prior to submission of bids; is verified for ownership and control, and is so listed in the Vendor Information Pages database, http://www.VetBiz.gov, if applicable. 5. Proposals will be considered only from offerors who are regularly established in the business called for and who are financially responsible. Offerors must have the necessary equipment and personnel to furnish services in the volume required for all items specified under this contract. 6. Successful offeror(s) shall meet all requirements of Federal, State and City codes regarding operation of this type of service or establishment. The contract personnel shall provide services in accordance with the terms, conditions and prices contained herein. Contract costs shall be all-inclusive, including but not limited to salaries, fringe benefits, vacation pay and malpractice expenses. 7. Electronic, hand-delivered or properly mailed offers will be accepted. Telephonic and faxed offers will not be accepted. The Government contemplates award of a firm, fixed price contract resulting from this solicitation. 9. Award(s) will be made to the offeror(s) whose offer represents the best value to the VA considering: technical capability, past performance and price. Technical and past performance or prior experience factors are considered equally important as price. 10. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation; will be most advantageous to the Government, price and other factors considered. The following information or factors shall be used to evaluate offers: Current Curriculum Vitae (CV) for each proposed provider (if applicable) Degrees earned, if any Copy of current license(s) for each proposed provider List of experience of each proposed provider References regarding related work with contact points and phone # Competency checklist for each proposed provider Rating factors for the evaluation of proposals will include: Availability and submission of the required information above Prior experience with the agency Price (Vendor shall include all-inclusive prices) 11. It is not possible at the time of solicitation to list all the possible CPT codes that may apply to this contract. Additional CPT codes may be added to the list of codes specified below or codes may be deleted during the course of this contract. Additions or deletions will be made by Supplemental Agreement(s) to this contract as needed.
