- SCHEDULE OF SUPPLIES/SERVICES: RADIOLOGY TECHNICAL COMPONENT This contract is for a base and 4 (12) month option periods for the Rutland Centrally Based Outpatient Clinic in Rutland, Vermont. VA Outpatient Clinic at Rutland, VT 232 West Street Rutland, VT 05701 The Contractor shall furnish all equipment, facility, and technical personnel services to provide routine x-ray films for eligible veteran beneficiaries as referred by the Community Based Outpatient Clinic's (CBOC) a part of VA Medical Center, White River Junction, VT. The Contractor's place of business must be in close proximity to the CBOC's (less than 5 miles) so as to facilitate timely patient care. The Contractor must possess all licenses, permits, accreditation's and certificates as required by law. Contract services will be provided on an as needed basis. The contract base year period will be from the date of the Award through September 30, 2012, subject to the availability of FY12 funding, with the option to extend the contract for four (4) one year option periods. It is the VA's intention to negotiate and award on a competitive basis, pending availability of funds. Eligible veteran beneficiaries will be directly referred from the CBOC to the contractor selected for Imaging Services. Patients will be referred on an as needed basis. Patients will be identified as eligible veteran beneficiaries by presenting a Veteran's Universal Access ID Card. The services rendered will be for the technical component only. All examinations will be interpreted by VAMC WRJ Radiologist's. (The facilities will not be held liable for any imaging studies that are not interpreted by their staff.) Patients will be walk in patients Monday thru Friday, between the hours of 8:00 am and 5:00 pm at the contractor's place of business. The patient will arrive at the contractor's place of business with a completed imaging order which will include the imaging requested, CPT code and appropriate clinical history. The order will be signed by the CBOC ordering clinician. Immediately upon completion of the imaging examination (not to exceed 6 hours) the contractor will send the images electronically via PACS to PACS connection to the White River Junction VAMC. During initial contract setup or network failure the vendor will provide all patient images on a CD stored in a DICOM (.dcm) format to the White River Junction VAMC within twenty- four (24) hours of the completion of the examination. The contractor will send CD's to White River Junction VAMC by certified mail, FedEx or UPS in order to provide a tracking number.
