This is a Source Sought Notice representing a market research survey and is not a Request for Quotation. NO SOLICITATION DOUMENTS EXIST AT THIS TIME. This Notice is to assist the VA in determining sources. Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub-zone, 8(a), Small Disadvantaged and Woman-Owned Small Business are encouraged to participate. This Sources Sought Market Survey is seeking to identify availability of radiopharmaceutical Aminophylline in the Phoenix, Arizona area. Businesses meeting all local, state, federal, industry, Joint Commission (JC), Nuclear Regulatory Commission (NRC), National Health Physics Program, Occupational Safety & Health Administration (OSAH) with the capability to furnish all labor, materials, supplies, equipment, transportation and qualified personnel to provide radiopharmaceutical aminophylline should respond. NAICS Code 325412 is proposed for this requirement. Responses must be emailed to kristin.tanner@va.gov no later than 4:00pm local time, September 21, 2012. Your response should include both the STATEMENT OF CABABILITY and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained below. Please place "Attention: Radiopharmaceutical Aminophylline" in the subject line of your email. This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Interested Vendor's shall include the following information in their responses: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates your company can provide the radiopharmaceutical. 3) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB), (g) State whether your firm is registered with the Central Contractor Registration (CCR) at http://www.ccr.gov and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE any future solicitation could only be awarded to a contractor who is registered in CCR and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. 4) Submit Capability Statements by September 21, 2012 to kristin.tanner@va.gove by 4:00 local time. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER September 21, 2012. 4) Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response.
