- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This Request For Quotes (RFQ) will also use the procedures under FAR Part 13. In accordance with 13.106-2, the contracting officer may take into consideration information such as the contracting officer's knowledge of and previous experience with the supply being acquired or other reasonable basis. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The reference/solicitation number is VA263-12-Q-1448 and the solicitation is issued as an RFQ. The combined synopsis/solicitation will result in a purchase order. (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-61. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR). (iv) This solicitation is posted as total small business set-aside. North American Industry Classification System (NAICS) code is 812332 (size standard is $35.5 Million) applies to this solicitation. (v) This requirement is for a temporary laundry service. The period of performance shall run from October 1, 2012 through December 28, 2012. The articles to be laundered include: "Spread blankets, bath blankets, fitted sheets, laundry bags, various sizes of pj's, patient t-shirts, sweat pants, IV gowns, patient gowns, towels, and washcloths. "Flat sheets, draw sheets, and pillowcases. "Surgery huck towels, a few surgery wraps, and impervious surgical gowns. "Various types of uniforms, (to include colored and white), scrubs, and lab coats. "Mops and rags. "Various sizes of privacy curtains The Fargo VA HCS shall deliver soiled linens and pickup processed linens the following day. Therefore, only offers submitted within a 10 mile radius of the Fargo VA HCS will be accepted. (vii) This section is not applicable (viii) The provision at FAR 52.212-1 Instructions to Offerors - Commercial Items, applies to this solicitation and incorporate the following addenda to 52.212-1, 52.204-6 Data Universal Numbering System (DUNS) Number; 852.233-70 Protest Content; 852.236-76 Correspondence. Offerors who respond to this requirement must provide, on a signed and dated company letterhead stationary, the following information in the response: "Price per pound "Location of facility "Copy of 52.212-3 - Offeror Representations and Certifications or validation company is current in ORCA (ix) The provision at FAR 52.212-2, Evaluation - Commercial Items applies to this requirement. The Government will award a firm fixed price purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award will be made on a low price, technically acceptable basis. (x) The provision at 52.212-3, Offeror Representations and Certification, Commercial Items, applies to this solicitation. The contractor shall either: (1) return a completed copy of this provision with its quote (a copy of the provision may be attained from https://www.acquisition.gov/far/); or (2) complete the Online Reps and Certs. Application (ORCA) at http://www.bpn.gov and reference it in your quote. (xi) The clause at 52.212-4 Contract Terms and Conditions - Commercial Items, applies to this acquisition and incorporate the following addenda to 52.212-4: 852.203-70 - Commercial Advertising, 852.273-76 - Electronic Invoice Submission (xii) 52.212-5 Contracts Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; The following FAR clauses identified at paragraph b of FAR 52.212.5 are considered checked and are applicable to this acquisition: 52.204-10 - Reporting Executive Compensation & First-Teir Subcontract Awards; 52.222-3 - Convict Labor, 52.222-19 - Child Labor-Cooperation with Authorities and Remedies, 52.222-21 - Prohibition of Segregated Facilities; 52.222-26 - Equal Opportunity; 52.222-36 - Affirmative Action for Workers with Disabilities, 52.223-18 - Encouraging Contractor Policies to Ban Text Messaging While Driving; 52.225-3 - Buy American Act - Free Trade Agreements - Israeli Trade Act, 52.225-13 - Restriction on Certain Foreign Purchases; 52.232-34 - Payment by Electronic Funds Transfer - Other than Central Contractor Registration. (xiii) To be awarded this contract, the offeror must be registered in the System for Award Management (SAM). SAM information may be found at https://www.sam.gov/portal/public/SAM/. (xiv) The Defense Priorities and Allocations System (DPAS) is not Applicable to this acquisition. (xv) Quotes are due September 21, 2012, at 3:00 PM EST.
