TEH REUQEST IS FOR TEN BIO-MED TECHNICIANS TO START ON 10-01-12 THIS IS A REQUEST FOR QUOTATION FOR BIOMED TECHNICIANS AS DESCRIBED IN THE SOW AND THE SELECTION AS PROVIDED UNDER EVALUATION FACTORS.STATEMENT OF WORK (SOW) STATEMENT OF WORK FOR (10) BIOMEDICAL REPAIR TECHNICIANS (BMET) 1.0 STATEMENT OF WORK 1.1 SCOPE Under this task order, the contractor will independently evaluate, maintain and repair medical and dental equipment at WRNMMC Bethesda in support of the treatment of patients, diagnosis of pathological conditions and in biomedical research. This employee may be required to perform in any assigned area of WRNMMC. 1.2 ORGANIZATION Walter Reed National Military Medical Center 8901 Wisconsin Avenue Bethesda, Maryland 20889-5600 The mission of Walter Reed National Military Medical Center is to: • Maximize operational readiness and keep the Uniformed Services mission-ready. • Serve as National Capital Regional Resource for homeland defense. • Provide quality primary care and specialty services in a caring, patient-centered environment. • Provide outstanding, customer-focused services for our Department of Defense family. • Provide distinguished graduate medical and dental education and ensure professional development for all staff members. • Develop and export innovations in health care, informatics and research. • Actively collaborate in an integrated NCA Health System. • Provide outstanding base operating support to tenant commands co-located on the WRNMMC compound. • Care for the Nation's leaders. • Serve as the President's Hospital. 2.0 TASK TYPE Time-and-Materials (T&M) 20,000 hours per year with an 8 hour work day. 3.0 DUTIES AND RESPONSIBILITIES 3.1 TASKS/SERVICES The contractor shall: 3.1.1 The contractor shall: Install, modify, troubleshoot, maintain, test, calibrate, adjust, overhaul and repair a wide variety of medical, laboratory, and dental equipment (electronic, electrical, and mechanical). Typical examples of equipment serviced include radiographic diagnostic and treatment units, cardiac defibrillators, hemodialysis units, ultracentrifuges, spectrophotometers, ultrasonic therapy equipment, electrophoresis apparatus, gas chromatographs, electron microscopes, X-ray units, electrocardiograph machines, cardiac monitors, electrosurgical units, electroencephalographs and radiography high speed film processors. The incumbent maintains and repairs chemical, mechanical, hydraulic, pneumatic, vacuum, suction and pressure systems which may include water, steam, air, gas under pressure, and piped oxygen lines associated with equipment serviced. 3.1.2 The contractor shall: Determines nature and extent of repairs required by troubleshooting equipment for malfunctions, requiring the tracing of wiring, replacing broken or worn parts, detecting leaks, feedback, and interference. Determine voltage tolerances, amperage, power supplies, wave shape, frequency resonance, deflection time, decay curve, gain, balance, and signal sensitivity and comparing against prescribed tolerances and specifications. The incumbent plans and carries out work assignments using blueprints, schematic drawings, technical manuals, wiring diagrams, liquid/air flow sheets, and other specifications. 3.1.3 The contractor shall: Test equipment after repair and also tests new equipment to assure proper functioning; conducts preventive maintenance inspection of equipment; performs periodic checks to ensure that equipment users are following proper preventive maintenance procedures; and conducts on-the-job training for all equipment users and biomedical repair technicians in the maintenance of medical, laboratory, and dental equipment. 3.1.4 The contractor shall: Will be assigned work either orally or through work orders. Incumbent independently determines the nature of trouble and extent of repairs required and completes repairs following established trade practices and procedures, using technical manuals, blueprints, sketches, wiring diagrams, liquid and air flow sheets, and other specifications. The incumbent performs operational and safety tests after repairs have been completed or when new equipment is received to assure that equipment is operating within prescribed limits so that lives are not endangered. The supervisor provides limited oversight and spot checks work for compliance with acceptable trade practices, directives, and operating specifications. The supervisor provides technical advice and assistance on unusual and more difficult problems. 4.0 ADMINISTRATIVE & TRAINING REQUIREMENTS 4.1 CERTIFICATIONS, LICENSE, PHYSICAL REQUIREMENTS OR OTHER EXPERTISE REQUIRED. 4.1.1 The contractor must possess the practical knowledge of the principles of electronic circuits, functions of electronic components, including X-ray tubes, in order to diagnose malfunctions, repair and test such electronic equipment as electrocardiographs, defibrillators, heart oscilloscopes, electron microscope, electroencephalographs, and spectrophotometers. 4.1.2 The contractor possess the knowledge of the relationships between electrical and electronic circuitry as well as the interrelationship of chemical, radiological, hydraulic, pneumatic, vacuum, or gas components of a piece of equipment or a system. 4.1.3 The contractor must possess a thorough knowledge of the principles and theories governing the maintenance and repair of electrical equipment in order to diagnose malfunctions, and repair and test such electrical equipment as sterilizers, film driers, suction apparatus, and dental units. Incumbent must apply knowledge of hydraulic systems including their grips, locks, stop and release mechanisms, and have the ability to determine proper adjustment with no leaks. 4.1.4 The contractor must possess the ability to repair mechanical equipment such as sterilizers, X-ray tables, operating tables, pressure breathing therapy units, respirators, and other mechanical assemblies using gears, shafts, and pulleys. Incumbent must also select proper replacement parts, modification kits, and identify materials, mechanisms, and components of electrical systems. The incumbent must possess a thorough knowledge of electronic symbols, color codes, descriptions, and manufacturer's numbers. The incumbent uses skills and precision in working with small components, requiring good hand and eye coordination and functional color vision. 4.1.5 The contractor must possess a thorough knowledge in the use of electronic test equipment such as digital multi-meters, oscilloscopes and signal generators. Incumbent must have the ability to use electrical test equipment such as meggers, ohmmeters and test meters. Incumbent must be skilled in the use of such equipment as micrometers and calipers to detect wear and misalignment of gears, cams, valves, and other mechanical parts; and ability to use seat wrenches and pressure gauges when working on hydraulic systems. 4.1.6 The contractor must possess the ability to interpret and apply blue prints, schematic drawings, wiring diagrams, liquid/air flow sheets, technical manuals, and other specifications. 4.1.7 The contractor must possess the knowledge to make various mathematical computations using algebraic expressions and shop formulas in relation to load requirements of wiring and equipment. 4.1.8 It is desirable that the contractor possess an associate's degree in Biomedical Engineering Technology or equivalent military training and at least 2 years experience in the maintenance, calibration, and repair of biomedical/dental systems. 4.1.9 It is desirable that the contractor is certified by the International Certification Commission as a Certified Biomedical Repair Technician (CBET). 4.1.10 The contractor must demonstrate familiarity and understanding of current technology changes in the biomedical engineering discipline. 4.1.11 Work assignments involve standing, stooping, bending and frequently working in cramped and awkward positions for prolonged periods of time. Movement of heavy equipment may be required. The employee must be able to lift 60 pounds and 10 - 40 pounds several times in succession. 4.1.12 Work is primarily performed in the Equipment Management Department, WRNMMC at a workbench. Other work areas includes the Radiology Service Line, Laboratory, ICU, NICU, MICC, MOR, the wards, any other areas within WRNMMC 4.1.13 The immediate supervisor is the BMET Supervisor of the Biomedical Repair Division. 4.1.14 The contractor operates under broad guidelines and general supervision and is technically responsible for the quality of work accomplished. The supervisor expects the employee to take independent action in solving problems that arise. 4.1.15 The employee functions individually in the performance of the duties and responsibilities of this position description, with supervisory controls exercised over the position being general in nature and usually dealing with matters related to overall objectives, policies and operations of the Biomedical Engineering Division. 5.0 PERIOD OF PERFORMANCE The period of performance includes the base year beginning 01 October 2012 and ending 30 September 2013. 6.0 PLACE OF PERFORMANCE The place of performance is the Walter Reed National Military Medical Center and any of the WRNMMC's. This includes commands supported by agreement such as the White House, the US Capital, Air Force One and Camp David. 7.0 OPERATIONAL HOURS Work will be performed up to 40 hours per week during normal business hours 0730 - 1530, Monday-Friday, excluding Federal holidays. 8.0 OVERTIME Overtime is not authorized. 9.0 SAFETY ISSUES The contract must wear electrically safety/steel toe boots. 10.0 GOVERNMENT-FURNISHED EQUIPMENT, BADGE, KEYS AND/OR FACILITIES PROVIDED All equipment necessary for the performance of all tasks will be provided by the Government. 11.0 SECURITY CLEARANCE/POLICE CHECK/DRIVING RECORD CHECK Work under this task order is UNCLASSIFIED. The contractor shall comply with all applicable Department of Defense (DoD) security regulations and procedures during the performance of this task order. The contractor shall not disclose and must safeguard procurement sensitive information, computer systems and data, privacy act data, and Government personnel work products that are obtained or generated in the performance of this task order. If necessary, the contractor will be required to provide clearances for personnel requiring access to Government computers and workstations. The contractor must wear a WRNMMC Bethesda ID badge issued by the WRNMMC Bethesda Pass/Id Department. 12.0 REIMBURSABLE EXPENSES (TRAVEL, CONFERENCE, TRAINING, EQUIPMENT OR MATERIALS, ETC.) Travel is required. The contractor may be required to travel to other locations in support of the tasks described in this statement of work. Prior to incurring any travel expenses, contractor personnel must obtain written authorization from the Biomedical Engineering Supervisor to approve of approximate travel, dates, expected duration, origin and destination, purpose, estimated costs and the number and names of personnel traveling. Contractor expense reports shall be prepared and processed in accordance with the Federal Travel Regulation (FTR). 13.0 NON-PERSONAL SERVICE STATEMENT Contracted employees performing services under this order will be controlled, directed and supervised at all times by management personnel of the contractor. The contractor's management shall ensure that employees properly comply with the performance standards outlined in the Quality Assurance Surveillance Plan (QASP). The Contractor shall provide a replacement for contractors that terminate their employment, transfer to another position or permanently leave the facility. Contractor employees will perform independent of and without the supervision of any Government official. Actions of contractor employees may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal policy, obligating the appropriated funds of the U.S. Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR). The Government will control access to the facility and will perform the inspection and acceptance of the completed work. As used in this clause: Individual has the same meaning as the term ``individual'' in 45CFR 164.501 and 164.103 and shall include a person who qualifies as a personal representative in accordance with 45 CFR 164.502(g).Privacy Rule means the Standards for Privacy of Individually Identifiable Health Information at 45 CFR part 160 and part 164, subparts A and E. Protected Health Information has the same meaning as the term ``protected health information'' in 45 CFR 164.501, limited to the information created or received by The Contractor from or on behalf of The Government. Required by Law has the same meaning as the term ``required by law'' in 45 CFR 164.501 and 164.103. Secretary means the Secretary of the Department of Health and Human Services or his/her designee. Security Rule means the Health Insurance Reform: Security Standards at 45 CFR part 160, 162 and part 164, subpart C. Terms used, but not otherwise defined, in this Agreement shall have the same meaning as those terms in 45 CFR 160.103, 164.501 and 164.304. (b) The Contractor agrees to not use or further disclose Protected Health Information other than as permitted or required by the Contract or as Required by Law. (c) The Contractor agrees to use appropriate safeguards to prevent use or disclosure of the Protected Health Information other than as provided for by this Contract. (d) The Contractor agrees to use administrative, physical, and technical safeguards that reasonably and appropriately protect the confidentiality, integrity, and availability of the electronic protected health information that it creates, receives, maintains, or transmits in the execution of this Contract. (e) The Contractor agrees to mitigate, to the extent practicable, any harmful effect that is known to the Contractor of a use or disclosure of Protected Health Information by the Contractor in violation of the requirements of this Contract. (f) The Contractor agrees to report to the Government any security incident involving protected health information of which it becomes aware. (g) The Contractor agrees to report to the Government any use or disclosure of the Protected Health Information not provided for by this Contract. (h) The Contractor agrees to ensure that any agent, including a subcontractor, to whom it provides Protected Health Information received from, or created or received by the Contractor on behalf of the Government agrees to the same restrictions and conditions that apply through this Contract to the Contractor with respect to such information. (i) The Contractor agrees to ensure that any agent, including a subcontractor, to whom it provides electronic Protected Health Information, agrees to implement reasonable and appropriate safeguards to protect it (j) The Contractor agrees to provide access, at the request of the Government, and in the time and manner designated by the Government to Protected Health Information in a Designated Record Set, to the Government or, as directed by the Government, to an Individual in order to meet the requirements under 45 CFR 164.524. (k) The Contractor agrees to make any amendment(s) to Protected Health Information in a Designated Record Set that the Government directs or agrees to pursuant to 45 CFR 164.526 at the request of the Government or an Individual, and in the time and manner designated by the Government. (l) The Contractor agrees to make internal practices, books, and records relating to the use and disclosure of Protected Health Information received from, or created or received by the Contractor on behalf of, the Government, available to the Government, or at the request of the Government to the Secretary, in a time and manner designated by the Government or the Secretary, for purposes of the Secretary determining the Government's compliance with the Privacy Rule. (m) The Contractor agrees to document such disclosures of Protected Health Information and information related to such disclosures as would be required for the Government to respond to a request by an Individual for an accounting of disclosures of Protected Health Information in accordance with 45 CFR 164.528. (n) The Contractor agrees to provide to the Government or an Individual, in time and manner designated by the Government, information collected in accordance with this Clause of the Contract, to permit the Government to respond to a request by an Individual for an accounting of disclosures of Protected Health Information in accordance with 45 CFR 164.528. General Use and Disclosure Provisions Except as otherwise limited in this Agreement, the Contractor may use or disclose Protected Health Information on behalf of, or to provide services to, the Government for the following purposes, if such use or disclosure of Protected Health Information would not violate the Privacy Rule, the Security Rule or the Department of Defense Health Information Privacy Regulation if done by the Government: [List Purposes]. Specific Use and Disclosure Provisions (a) Except as otherwise limited in this Agreement, the Contractor may use Protected Health Information for the proper management and administration of the Contractor or to carry out the legal responsibilities of the Contractor. (b) Except as otherwise limited in this Agreement, the Contractor may disclose Protected Health Information for the proper management and administration of the Contractor, provided that disclosures are required by law, or the Contractor obtains reasonable assurances from the person to whom the information is disclosed that it will remain confidential and used or further disclosed only as required by law or for the purpose for which it was disclosed to the person, and the person notifies the Contractor of any instances of which it is aware in which the confidentiality of the information has been breached. (c) Except as otherwise limited in this Agreement, the Contractor may use Protected Health Information to provide Data Aggregation services to the Government as permitted by 45 CFR 164.504(e)(2)(i)(B). (d) Contractor may use Protected Health Information to report violations of law to appropriate Federal and State authorities, consistent with 45 CFR 164.502(j)(1). Obligations of the Government Provisions for the Government to Inform the Contractor of Privacy Practices and Restrictions (a) Upon request the Government shall provide the Contractor with the notice of privacy practices that the Government produces in accordance with 45 CFR 164.520, as well as any changes to such notice. (b) The Government shall provide the Contractor with any changes in, or revocation of, permission by Individual to use or disclose Protected Health Information, if such changes affect the Contractor's permitted or required uses and disclosures. (c) The Government shall notify the Contractor of any restriction to the use or disclosure of Protected Health Information that the Government has agreed to in accordance with 45 CFR 164.522. Permissible Requests by the Government The Government shall not request the Contractor to use or disclose Protected Health Information in any manner that would not be permissible under the Privacy Rule if done by the Government, except for providing Data Aggregation services to the Government and for management and administrative activities of the Contractor as otherwise permitted by this clause. Termination (a) Termination. A breach by the Contractor of this clause, may subject the Contractor to termination under any applicable default or termination provision of this Contract. (b) Effect of Termination. (1) If this contract has records management requirements, the records subject to the Clause should be handled in accordance with the records management requirements. If this contract does not have records management requirements, the records should be handled in accordance with paragraphs (2) and (3) below (2) If this contract does not have records management requirements, except as provided in paragraph (3) of this section, upon termination of this Contract, for any reason, the Contractor shall return or destroy all Protected Health Information received from the Government, or created or received by the Contractor on behalf of the Government. This provision shall apply to Protected Health Information that is in the possession of subcontractors or agents of the Contractor. The Contractor shall retain no copies of the Protected Health Information. (3) If this contract does not have records management provisions and the Contractor determines that returning or destroying the Protected Health Information is infeasible, the Contractor shall provide to the Government notification of the conditions that make return or destruction infeasible. Upon mutual agreement of the Government and the Contractor that return or destruction of Protected Health Information is infeasible, the Contractor shall extend the protections of this Contract to such Protected Health Information and limit further uses and disclosures of such Protected Health Information to those purposes that make the return or destruction infeasible, for so long as the Contractor maintains such Protected Health Information. Miscellaneous (a) Regulatory References. A reference in this Clause to a section in the Privacy Rule or Security Rule means the section as in effect or as amended, and for which compliance is required. (b) Survival. The respective rights and obligations of Business Associate under the ``Effect of Termination'' provision of this Clause shall survive the termination of this Contract. (c) Interpretation. Any ambiguity in this Clause shall be resolved in favor of a meaning that permits the Government to comply with the Privacy Rule or Security Rule. EVALUATION FACTORS EVALUATION FACTORS THE CONTRACT AWARD WILL BE MADE THE PROSPECTIVE CONTRACTOR THAT MEETS THE BEST VALUE AND COST SUB-FACTORS. BEST VALUE FACTORS: The contrator that offers the highest points in best value on the follwing facts will be deemed the most preferred quote. 1. Personnel training and qualifications assessment- the Biomedical Equipment Technicians(BMET) must have some formal college education such as some college or military trainign in the subject field. Skill and experience in DMLSS and CBET are preferable. 2. Past performamnce- the contractor should have at least two years of performance in biomed technical equipment contract, 3. Performance Measures -The ability or Capability to start Immediately with at least two BMET at the starting period of the contract, 4. Contractor should should provide a lead BMET that serves as a team leader that will be responsiblefor for company specific administrative issues on a daily basis, 5. The cost of a BMET shall be based on the on going labor market rate or according to Department of labor wage rates. Compensation or cost per bio-med equipment technicia will be at grade GS-10/11.
