- Industrial Hygiene Surveys CDRLs Revised PWS FA8224-12-R-0089-0003 Conformed Solicitation This is a combined synopsis/solicitation for commercial items/services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Request for Proposal (RFP) number is FA8224-12-R-0089. The Government contemplates award of one (1) contract resulting from this solicitation. This acquisition is for the procurement of Calibration and Emergency Maintenance Services on Thermal Spray Equipment within the 532nd Commodities Maintenance Group, Hill Air Force Base, UT. See attached solicitation, Performance-Based Work Statement (PWS) dated 5 June 2012, CDRLs, and Industrial Hygiene (1/H) Surveys for full details of contract requirements. NOTE: Government provided I/H Surveys are for informational purposes and cannot be used by the contractor for his own occupational health compliance purposes. Contractors will be required to perform IH surveys as necessary and are advised to propose accordingly. Anticipated period of performance is one (1) year with two (2) option years. ANTICIPATED AWARD DATE: The contract is expected to be awarded on or about, 31 October 2012. Offerors are advised to review 52.212-1 Instruction to Offerors and 52.212-2 Evaluation, clauses in the solicitation for instructions to Offerors and evaluation criteria.
