Memorandum For Record Subject: Solicitation VA244-12-Q-1865 Department of Veteran Affairs Lebanon, PA This solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. There is only one responsible source and no other services will satisfy agency requirements and IAW FAR 6.302-1 it is being procured as a sole source acquisition. This requirement is being filled utilizing NAICS 339113 with a size standard is 500 employees. The Federal Supply Class (FSC) is 6515 and the Standard Industrial Classification (SIC) is 3842. This acquisition uses a brand name description and purchase description that specifies a particular brand name, product, along with features of a product that is particular to Cadwell Laboratories, Inc., 909 North Kellogg Street Kennewick, WA 99336. This award is anticipated for September 20120 and the following items are being procured: (2) Two Sierra wave 2 channel system (2) Two EMG/NCV/SEP/F-wave/H-rflx/RNS, Blink reflex (2) Two Stimtroller (2) Two Soft Carry Case with wheels & handle (2) Two Patch Cable for Laptop Computer 2' (2) Two BBB Electrode Kit (2) Two Microsoft Office 2010 (2) Two USB Mouse (2) Two Skin Temp Probe CE (2) Two DeskJet Printer (110v) (2) Two Point of Care Cart (Laptop computer only) (2) Two Wave Arm w/bracket for Stim & Amp IAW FAR 13.104, the Contracting Officer has taken all steps practical to promote competition to the maximum extent in order to ensure an acquisition that is most advantageous to the government. After a complete review of this requirement, it has been determined that soliciting from a single source as directed by FAR 13.106(2) (b) (1) (I) is warranted by this proposed contract action. This acquisition requires brand name items from Cadwell Laboratories, Inc. and appropriate justification and approval is incorporated. Because the instruments being procured are currently being tested in Hines VA interfacing, it will facilitate an error reduction in transcribing patient files that could have catastrophic effect on patient well being. Based on this fact and the determination that the quote from Cadwell Laboratories is fair and reasonable, the Contracting Officer has determined that it would be reasonable to go directly to one source. Due to the sensitive nature of the equipment being procured, it has been determined that disclosure of the brand name and sole source justification could potentially create a security risk for the agency and therefore will not be made publically available as required by FAR 13.501(a).
