- This is a Sources Sought Notice (Request for Information). The Government intends to establish a sole source Blanket Purchase Agreement (BPA) to Psychological Assessment Resources, Inc.(PAR, Inc.) for psychological testing/assessment materials necessary to assess and diagnose patients, develop treatment plans, and evaluate the effectiveness of treatment for the Department of Psychology at Tripler Army Medical Center, Hawaii. Calls will be placed against the BPA as supplies are required. Period of performance will be for five years. NAICS code is 511199. See attached list of items required. Interested parties shall reply to Pacific Regional Contracting Office VIA EMAIL to Annette.Pickering@us.army.mil. TELEPHONE REQUESTS WILL NOT BE HONORED. Please indicate your interest and/or ability to provide the attached list of items and also provide a current copy of your price list for the items and your capabilities statement NO LATER THAN 19 October, 12:00 P.M. Hawaii Standard Time or 6:00 PM Eastern Time. Vendor MUST BE ABLE TO PROVIDE 100% of the item(s) listed to be considered for this BPA.
